For more details, please visit http://www.nemocheung.com ANNEX The Lockout/Tagout Program is intended to ensure that all personnel are protected from accidental or unexpected activation of mechanical and/or electrical equipment during maintenance, repairing, cleaning, servicing, or adjusting of prime movers, machinery, or equipment. The program outlines the purpose, authorization and practices to be utilized by COMPANY . (COMPANY) personnel and contractors to guard against the unexpected energizing, start-up, or release of stored energy that could cause injury. (If personnel are servicing or performing maintenance tasks that do not expose them to the potential for unexpected release of hazardous energy, these program requirements do not apply) Energy sources include mechanical, electrical, hydraulic, pneumatic, chemical, thermal or other energy. Personnel performing servicing or maintenance activities or operations are covered by lockout/tagout when any of the following conditions occur:  The employee must either remove or bypass machine guard(s) or other safety device(s), resulting in exposure to hazards at the point of operation;  The employee is required to place any part of his/her body in contact with the point of operation of the machine or piece of equipment; or  The employee is required to place any part of his/her body into a danger zone associated with a machine operating cycle. In the above situations, the equipment must be de-energized and locks and tags must be applied to the energy isolation devices. The program does not apply in the following situations:  While servicing or maintaining cord and plug connected electrical equipment. (The hazards must be controlled by unplugging the equipment from the energy source; the plug must be under the exclusive control of the employee performing the service and/or maintenance.)  During hot tap operations that involve transmission and distribution systems for gas, steam, water, or petroleum products when they are performed on pressurized pipelines; when continuity of service is essential, and the shutdown of the system is impractical; and employees are provided with an alternative means of protection. Employees performing minor tool changes or adjustments and/or other minor service activities during normal production operations that are routine, repetitive, and integral to the use of the equipment are not covered by the lockout/tagout program, provided the work is performed using alternative measures that give effective protection. Affected Employee: An employee whose job requires them to operate or use a machine or piece of equipment on which servicing is being performed under lockout or tagout, or whose job requires them to work in an area in which such servicing or maintenance is being performed. Authorized Employee: A person who locks or implements a tagout system procedure on machines or equipment to perform the servicing or maintenance on that machine or equipment. An authorized employee and an affected employee may be the same person when the affected employee's duties also include performing maintenance or service on a machine or piece of equipment that must be locked, or a tagout system implemented. Energy Source: Any source of electrical, mechanical, hydraulic, pneumatic, chemical, thermal, or other energy. Lockout: The placement of a lockout device on an energy-isolating device, in accordance with an established procedure, ensuring that the equipment being controlled cannot be operated until the lockout device is removed. For more details, please visit http://www.nemocheung.com ANNEX Lockout Device: A device that utilizes a positive means, such as a lock, to hold an energy- isolating device in the safe position and prevent the energizing of a machine or piece of equipment. Servicing and/or Maintenance: Workplace activities such as constructing, installing, setting up, adjusting, inspecting, modifying, and maintaining and/or servicing machines or equipment. These activities include operations where the employee may be exposed to the unexpected energization or start-up of the equipment or release of hazardous energy. Tagout: The placement of a tagout device on an energy- isolating device, in accordance with an established procedure, to indicate that the equipment being controlled may not be operated until the tagout device is removed. The established procedure for the application of Lockout/Tagout shall cover the following elements and actions:  Preparation for lockout/tagout  Sequence for lockout or tagout  Restoring machines or equipment to normal operations  Lockout/tagout involving more than one person  Temporary removal of lockout/tagout devices Implementation of the lockout or tagout system shall be performed only by authorized employees. Affected employees shall be notified by the authorized employee of the application and removal of lockout or tagout devices. Notification shall be given before the controls are applied, and after they are removed from the machine or equipment. PREPARATION FOR LOCKOUT/TAGOUT Make a survey to locate and identify all isolating devices to be certain which switch(es), valve(s), or other energy isolating devices apply to the equipment to be locked or tagged out. More than one energy source (electrical, mechanical, stored energy, or others) may be involved. SEQUENCE OF LOCKOUT OR TAGOUT 1. Notify affected personnel that a lockout or tagout system is going to be utilized and the reason therefore. The authorized employee shall know the type and magnitude of energy that the machine or equipment utilizes and shall understand the hazards thereof. 2. If the machine or equipment is operating, shut it down by the normal stopping procedure (depress stop button, shut valve, etc.). 3. Operate the switch, valve, or other energy isolating device(s) so that the equipment is isolated from it energy source(s). Stored energy (such as that in elevated machine members, rotating flywheels, hydraulic systems, and air, gas, steam, or water pressure, etc.) must be dissipated or restrained by methods such as repositioning, blocking, bleeding down, etc. 4. Lockout/Tagout the energy isolating devices with assigned individual lock(s) or tag(s). 5. After ensuring that no personnel are exposed, and as a check to ensure disconnection of the energy sources, operate the push button or other normal operating controls to make certain the equipment will not operate. CAUTION: Return operating control(s) to neutral or off position after the test. 6. The equipment is now locked out or tagged out. RESTORING MACHINES OR EQUIPMENT TO NORMAL OPERATIONS 1. After the servicing and/or maintenance is complete and equipment is ready for normal production operations, check the area around the machine or equipment to ensure that no one is exposed. After all tools have been removed from the machine or equipment, guards have been reinstalled and employees are in the clear, remove all lockout or tagout devices.
