本期《临床肿瘤学杂志》为妇科肿瘤专题,特别是卵
巢上皮癌治疗的新进展。在过去三十年里,卵巢癌患者的
生存率呈缓慢但稳定的上升趋势。尽管泰素联合铂类静脉
化疗方案提高了新诊断的晚期患者的生存率,但大多数人
仍不能治愈[1]。近期,三项随机试验证实,对于新诊断的III
期卵巢上皮癌患者,在满意的减瘤术后,顺铂腹腔内(IP)化
疗较全身化疗能带来更多的受益[2]。其中一项研究显示,
与紫杉醇+顺铂(IV)方案对照组相比,紫杉醇(IV,d1)+顺铂
(IP,d2)+紫杉醇(IP,d8)方案使患者的中位总生存期延长了
16个月[3]。然而,相当一部分患者(58%)因为各种毒性反应
而无法完成这种化疗,包括腹腔导管感染和堵塞、中性粒
细胞减少性发热以及神经毒性等。Rao等[4]的报道详细讨论
了IP疗法,并回顾了减少毒性反应和提高疗效的

此,将IP用药由顺铂改为卡铂、减少顺铂IP用药剂量以及将
贝乏单抗加入联合一线IP治疗等成为目前正在研究的改进

疗效并未得到证实的群体。这些患者包括IV期患者、减瘤

者。另外,IP治疗对于一些减瘤术满意但伴有神经病变,肾
病或广泛肠粘连等合并症的III期患者可能也不适合。
新诊断的卵巢癌患者治疗包括减瘤术和化疗。正如
Fader等[6]的综述中讨论到的,我们的妇科肿瘤同仁们一直致
力于这类患者的初始治疗。减瘤术满意(目前定义为残余病
灶直径小于1 cm)与生存的改善相关[7]。减瘤术是否仅适合
对术后化疗更敏感的患者,或减瘤术本身是否也有治疗作
用,这很可能是一个永远无法回答的问题。Fader等讨论了
手术在卵巢癌治疗中的重要作用,并评价了二次减瘤术在
复发患者治疗中的正确地位。
目前看来,进一步提高晚期卵巢癌患者的预后已不能
单靠传统的细胞毒性化疗方法。最近研究证实三药联合化
疗并不优于紫杉醇联合卡铂双药方案,这值得大家关注[8]。
基于此,我们在本期推出了三篇文章分别讨论免疫治疗[9]和
抗血管形成的新靶向治疗[10,11]。这些作者分别就相关领域讨
论了令人兴奋的进展,包括妇科肿瘤组(GOG)218等旨在探
讨贝乏单抗在一线及维持治疗中的作用的随机试验。最近
证实单药贝乏单抗伴有11%的肠穿孔率,其应用热度因而有
所下降,但该药仍显示对复发卵巢癌患者有效[12]。现已发
现既往细胞毒性化疗方案的数量和已伴有肠梗阻可能是这
一并发症的高危因素,不过我们仍需改进识别高危患者的
方法。除了抗血管形成药物外,新型靶向药物如聚ADP核
糖聚合酶等很有希望用于存在BRCA-1 或BRCA-2种系突变
的复发患者[13]。
并非所有卵巢癌患者预后都很差。在上皮癌中,早期
病变常常可通过单独手术或手术序贯辅助化疗治愈。Trope
等[14]讨论了早期卵巢癌的治疗措施及选择什么样的患者接
受辅助化疗。阻止卵巢细胞恶变是治疗卵巢上皮癌的最
可靠的措施。Kauff等 [15]回顾卵巢切除术减危治疗在伴有
BRCA-1或BRCA-2种系突变患者中的作用。交界性卵巢肿
瘤是非侵袭性卵巢肿瘤中值得关注的一类,通常为早期、
对化疗耐药,但一般预后较好。Cadron等[16]回顾了这类肿
瘤,分析了更保守的手术在渴望保留生育功能的年轻女性
患者中的作用。需要重点指出的是,有一类交界性肿瘤具
有侵袭性生物学行为,复发率和向低分级浆液性腺癌转化
率均较高[17]。这种交界性肿瘤的病理特征为通常存在特征
性微乳头结构及初始诊断时即有浸润表现。Gershenson[18]和
Colombo[19]分别回顾了其他来源于生殖细胞或性索间质的卵
巢癌。这种非上皮性卵巢癌通常治愈高率并对铂类敏感。
卵巢生殖细胞肿瘤尤其如此,大部分患者较为年轻,合适
的患者可以通过保留生育功能的手术序贯术后化疗而获得
有效的治疗。
其他妇科肿瘤的进展包括宫颈癌和高危子宫癌的治
疗。Monk等[20]详细回顾了铂类化疗联合放疗在局部晚期宫
颈癌中的治疗现状。转移性宫颈癌的治疗进展比较有限,
但一项研究显示拓扑替康联合顺铂可以改善一些转移性宫
颈癌患者的生存率[21]。Long[22]在报道中详细回顾了该方案
和其他方案在转移性宫颈癌中的作用。或许最令人兴奋的
进步是针对HPV血清型16和18 (二者是70%侵袭性宫颈癌的
病因)宫颈癌预防性疫苗的问世。Chan等[23]回顾了该疫苗的
疗效和适应证。鉴于侵袭性宫颈癌在全球特别是在发展中
