Hemorrhagic Shock Due to Consensual Coital Vaginal Laceration ??? ??? ????????? ???? Purpose: To demonstrate a severe and rare complication of voluntary coitus and search for the possible mechanisms of this unique gynecologic emergency. Case: There was a newly married female presented in our emergency room because of significant shock. Severe bleeding was found soon during her coitarche. There was no vaginismus and no any sex toy had been used in this case. When she was sent to our emergency room, hemoglobin was 5.5Gm/dL and blood pressure was 80/50. After primary repair of the vaginal lacerations and vigorous transfusion, the life of this female was saved. During operation, lacerations were found at posterior fornix and right lateral wall of vagina, with active bleeding of a ruptured artery. Conclusions: Vaginal laceration causing hemorrhagic shock in a voluntary coitus is a rare coital injury. Most of the reported cases in literature occurred among adolescence. Among those women mature enough, it occurred only in parous ones. It is severe and maybe fatal because of frequent ignorance by patient herself. Fear of coitus and following vaginismus is the main factor to cause this unique complication. Most of the sites of this kind of injury are located at posterior fornix and lateral wall, and there are anatomical factors. How to help these females relax themselves before coitus is the main goal to prevent this kind of coital injury.
