生育技术服务质量管理规范 第一章 总则 1.为了加强计划生育技术服务质量管理～规范计划生育技术服务行为～提高技术服务质量和水平～为人民群众提供优质、安全、满意的计划生育技术服务～根据《中华人民共和国人口与计划生育法》、《计划生育技术服务管理条例》等法律、法规及有关规定～制定计划生育技术服务质量管理规范。 2.计划生育技术服务要坚持安全第一、质量第一的方针。坚持以人为本～以服务对象为中心～遵循国家相关法律、法规。 3.计划生育技术服务质量规范管理目标是规范计划生育技术服务～提高计划生育技术服务质量～提升计划生育技术服务管理水平～保障服务对象的身心健康与生命安全。 4.计划生育技术服务机构和技术人员向服务对象提供服务时～必须保障服务对象的计划生育生殖健康权益～保障服务对象获得国家规定的免费技术服务～满足服务对象计划生育生殖健康及相关方面的需求。 5.本规范是各级计划生育技术服务机构开展计划生育技术服务工作和技术服务质量管理的基本要求和行为准则。 第二章 质量管理基本要求 1.计划生育技术服务质量规范管理包括机构管理、人力资源管理、基础设施管理、器械仪器设备管理、药品和避孕药具管理、技术服务过程管理、技术服务文书档案管理、环境和质量管理持续改进等。 计划生育技术服务机构应设立技术服务质量管理组织～配备技术服务质2. 量管理人员～制定技术服务质量#管理制度#～建立技术服务质量管理机制。计划生育技术服务机构应逐步引入现代质量管理方法～创新计划生育优质服务理念。 3.计划生育技术服务机构的法定代表人是技术服务质量管理的第一责任人～负责本机构技术服务质量管理组织的建立和质量管理、规范的组织实施.各部门 和各岗位人员是本机构实施全面质量管理的重要成员～是技术服务质量管理规范的实施者。 4.计划生育技术服务机构应建立和完善质量监督、考核评估、信息反馈机制。对工作结果进行记录、统计、分析、评估～并不断修正技术服务质量管理标准体系～保 持技术服务质量持续改进。 第三章 人员设施管理 (一)人力资源管理 1.计划生育技术服务机构应根据服务人群和机构规模～依照工作目标和质量控制 指标～制定发展规划～完善管理制度～明确人力资源管理的内容、职责、权限和考核、 奖惩办法～建立竞争、激励机制。 2.计划生育技术服务机构应根据有关法律法规、工作任务和服务项目设置岗位～确定岗位任职条件。岗位任职条件包括执业资格、受教育程度、业务培训、技能和经验等。 计划生育技术服务机构中技术人员的学历、职称、医技医护专业结构比例要与技 术服务工作任务和持续发展相适应～技术服务人员应占机构职工总数的80%以上。 3.计划生育技术服务机构中从事计划生育技术服务的人员必须取得《计划生育技术服务人员合格证》;凡是开展手术的人员～还应取得执业医师资格(注册执业医师、 注册执业助理医师等) ;从事护理、检验、药剂、影像、心电等相关临床服务的其他技术人员～还应具备相应的执业资格(如注册护师、注册检验技师、注册药师及其他相应的执业资格)和本机构上岗资格。不得聘用不具备相关资格的技术服务人员到相关的技术服务岗位从事技术服务工作。 4.计划生育技术服务机构应实行全员聘用、竞争上岗的用人制度。从事技术服务的人员必须是该机构的正式聘用人员进修和实习人员应在本机构技术服务人员的指导下进行工作。 5.计划生育技术服务机构按照国家人口和计划生育委员会《计划生育系统专业技术人员继续教育实施试行办法》 (国计生厅发[2001]176号)～实施继续教育培训制度。根据计划生育技术服务机构的工作任务和人员学历、职称～制订本机构的人力资源发展规划和中长期培训计划～通过考试、考核、考评等定期对培训进行评估和效果。 计划生育技术服务人员继续教育培训内容包括法律法规、政策、职业道德、规章制度、技术质量标准、操作规范及其他业务知识、专业技能等。 、培训方式包括脱产学习和在职教育培训两种方式。脱产学习包括学历培训|临床进修、短期培训等。在职教育培训包括短期集中培训"、专题讲座、函授、自学等。 6.加强计划生育技术服务机构技术人员职称管理～将技术人员的职称晋升纳入人力资源的发展规划～为技术人员的职称晋升创造条件。 (二)基础设施管理 1.计划生育技术服务机构应在建筑的外部环境、园林环境、室内环境等方面进行 统一、规范的设计和规划～营造舒适、洁净、优雅的环境。结合当地自然气候条件～ 选择适宜树种、花草进行绿化。室内环境色彩的搭配与室外色彩和谐统一～室内空间 布置要符合人们各种活动与审美的需要。 2.计划生育技术服务机构应根据服务半径、工作任务和服务项目设置～配备符合本规范要求的房屋、设备、药械、人员和业务科室等。新建、改建、扩建的县计划生 育服务站、中心乡(镇)计划生育服务站和普通乡(镇)计划生育服务站基础设施建设应当符合《农村计划生育服务机构基础设施建设标准》。 3.计划生育技术服务机构的科室设置应布局合理～以满足技术流程、保证服务质 量、方便群众为原则～因地制宜地划分为宣教候诊区域、咨询诊断区域、辅助检查区域、手术区域、观察康复区域及后勤供应区域等。 手术室、妇检室、检验室、X线室、消毒供应室等建筑应符合《综合医院建筑设计规范》。 4.宣教候诊区域:设立服务大厅和服务台。服务大厅应设有候诊椅、电视机、宣传栏和饮水处等公共设施。服务大厅的内部环境、空气质量、噪声、光照度应符合《医院候诊室卫生标准? ～并按照《公共场所卫生监测技术规范》适时进行监测检验。 5.咨询诊断区域:接诊的各科室应设在适当的位置～保持整洁、安静、安全和空气清新～采光、色彩应符合卫生学要求～并配备有专用流水洗手设备和符合 服务要求 医务的器械装备。 接诊技术科室应该布局合理～流程规范～卫生、整洁、温馨～符合《医院感染管理规范》、《消毒技术规范》及其他规范的要求。医疗仪器、设备、用品摆放整齐～无菌物品专柜存放～污染物品限区放置～及时处理;咨询室和特色咨询(悄悄话)室应具备能为不同人群提供咨询服务的条件～配备与咨询活动有关的设施、图片资料～布置温馨、隐秘～具有安全感。 咨询室应配备桌椅、书柜、药柜及简单的检查用具如血压计、体重称等～有各种 避孕药具的实物展板、使用方法说明和科普知识读物、挂图和模型～有人口和计划生 规章育政策法规方面的宣传资料。 妇检室的位置应与女性诊室相连或相邻;窗户应是带有纱窗的双层毛玻璃～地面采用坚硬、防滑、易清洗的材料～墙面和天花板应采用光滑、易清洁的材料～有取暖、 降温和洗手设备。女性治疗室应与妇检室分设。 6.辅助检查区域:检验室应开设两个窗口～分别用于采集标本和发放检验报告 单:检验台面应采用耐腐蚀、易冲洗、阻燃的材料;检验室的洗涤池和排水管应采用 耐腐蚀材料～配备消毒和通风设备。 放射室机房的设置必须考虑邻室和周围场所的防护与安全～一般应设在建2筑物底 层的一端;单管头200mA及以上X射线机房面积应?24m～双管头的宜?36。摄影机房中有用线素朝向的墙壁应有2mm铅当量的防护厚度～其他侧墙壁及透视机房各侧墙壁应有Imm铅当量的防护厚度。天棚、 考虑～其建筑设计应符合GB18871 - 2002 地板、门、窗、屏蔽等应视为相应侧墙壁 《电离辐射防护与辐射源安全标准》。 7.手术区域:手术室应设有准备间、更衣间、缓冲间、洗手间4个功能明确的房 间或区域(有条件的可单设污物间) ;手术室和相应的房间分别设定为无菌区、清洁 区、污染区3个不同的区域～并有明显的标记;将进入子术室的路线分别设立医护人 员通道、受术者通道和污物通道。 手术室的位置成选择采光和通风良好～远离厕所、牲畜圈、禽类窝、粉尘、噪声、 强光、街道、公路及人员流动较多的地方～相通的房间要安装双弹簧活动门或电动门～ 窗户应安装带有纱窗的双层毛玻璃～建筑材料隔音、防火、防湿～墙面、天花板、地 面应光滑、耐腐蚀、易清洁～呈浅色～地面应防滑～墙内角呈圆形以防止结网或集尘; 应有取暖、降温设备和急救药品、设备。 手术室洗手间应安装非子动水龙头～水龙头与洗手面的距离应有35厘米- 40 厘米。 乡(镇)计划生育技术服务机构的手术间和县级以上计划生育技术服务机2构的门 诊子术间面积应不低于16m～县级以上计划生育技术服务机构的大于术2间面积应不低于24m 0 8.消毒供应室应分为污染区、清洁区、无菌区和工作人员更衣室。消毒供应室应通风采光良好～周围无污染源;墙壁及天花板应无裂隙、不落尘～易于清洗和消毒; 地面光滑～有排水道。消毒供应室的物品进、HJ实行污、洁分道～单向通行～禁止逆 行;应有物品的接收、洗涤、晾晒、敷料制作、消毒灭菌、无菌存储、发放登记处。使用后的医疗物品按照接收?无害化处理?粗洗?精洗?打包?高压消毒?元菌物品 储存室?发放窗口的路径和流程运行。 9.观察康复区域:康复室和观察室应光线充足、通风良好～色调淡雅、柔和、2温 馨～内设卫生间或能方便上卫生间。康复(观察)室平均每床建筑面积6m _ 28m～ 窗地比应不低于1,7～设置通风、换气设施和调温设备。 10.后勤供应区域:仓库应设置有防腐、防虫、防鼠、防火、防盗、防潮设施。药品和避孕药具仓库还应有温、湿度计～以及温、湿度控制装置。 给水系统应该纳入城镇供水系统;生活饮用水管道应避开污染区～不能避开的应 采取防护措施～其水质应符合《生活饮用水卫生标准》,GB5749 – 1985)的规定; 给、排水系统应有防渗漏、防腐蚀、防堵塞措施～设置检修阀门方便适时检修;技术 服务人员使用的洗涤地、洗手池、化验清洗池等应采用非于动开关;公共卫生间内应 采用非于动冲水开关。 计划生育技术服务机构建筑的供电设施要采用双路电源;不具备双路电源的～应配备自动控制应急照明系统和应急电源～保证手术室、检验室、消毒供应室等重点科室的用电需要。 (三)器械仪器设备 1.计划生育技术服务机构应按照国家人口和计划生育委员会《计划生育技术服务机构设置标准》配置器械仪器设备。 2.计划生育技术服务机构应分别对器械仪器设备的采购、管理、使用、维护、校准和更新等进行质量控制。 3.计划生育技术服务机构要加强对采购器械仪器设备的管理～器械仪器设备采购要编制采购计划和采购方案～进行可行性论证～通过招标采购。采购要在资质符合国 家法律法规、行业标准～ 《企业法人营业执照》、《医疗器械经营企业许可证》、《医疗器械进口许可证》、《医疗器械产品注册证》等证照齐全的企业采购。企业销售的器械仪器设备应在其经营范围内～且有《产品检验合格证》。 采购的器械仪器设备要按照购货、产品合格证、包装、数量、规格、标识、产品说明书、发票等进行验收。 4.计划生育技术服务机构采购的器械仪器设备在使用前～应由供货方、质检人 员、使用操作人员进行安装、调试正常～操作人员掌握设备性能、操作方法、注意事 项等知识技能后～方可正式交付使用。 特殊精密贵重仪器设备～使用人员还应经过系统的理论学习和操作培训～考试考核合格后方可上岗操作。 5.计划生育技术服务机构要建立器械仪器设备保养维护制度～做好器械仪器设备的清洁、消毒、防锈等日常保养和维护O仪器设备发生故障时～应请专业维修人员进行维修～并作好保养、维修记录O对国家相关部门已经公布淘汰的仪器设备或未达到国家计量标准又不能校正修复的、超过使用期限的器械仪器设备～应该及时申请报废。 6.计划生育技术服务机构要做好仪器设备校准工作O凡列入国家《强制检定 作计量器具日录》的仪器设备～应定期或周期进行检定、校准～合格后方的工 可使用;仪 器设备在储存、使用和搬运前和维护后应对其进行校准检定。 7.计划生育技术服务机构应建立和完善器械仪器设备的管理制度。明确器械仪器 设备的管理部门、责任科室和责任人～加强会计核算～建立器械仪器设备档案。大型 的精密贵重仪器设备应确定专人管理。管理人员工作变动时应在相关部门负责人监督 下办理移交手续。 (四)药品和避孕药具管理 1.计划生育技术服务机构应建立药事管理组织～参《医疗机构药事管理办法》 ～ 建立健全药品和避孕药具管理制度～做好药品和避孕药具的计划编制、采购、验收、 人库、保管和使用的监督。 2.计划生育技术服务机构应按照需求编制药品采购计划～采购药品应严格执行国 家法律、法规和行业标准～采购方式符合国家相关规定。在符合资质并持有《药品经 营许可证》和《企业法人营业执照》的药品经营企业采购O所采购药品必须有国家药 品批准文号和产品检验合格证明。 采购进口药品必须在持有《进口药品注册证》或者《医药产品注册证》的药品经 营企业采购有《进口药品批件》、《进口药品检验报告书》或者注明"己抽样"并加 盖公章的《进口药品通关单》的进口药品。 采购生物制品必须在取得《生物制品生产合格证》的企业采购有药检部门《药品 检验报告书》的生物制品。 采购麻醉药品、精神药品等还应经药品监督管理部门批准～由专人负责到指定的 药品经营企业采购。 3.计划生育技术服务机构应建立采购药品验收制度。由专门的部门和人员对采购 的药品依据采购计划、产品合格证、生产日期和有效日期、包装、数量、规格、标识、 产品说明书、发票等进行验收。 4.计划生育技术服务机构应建立健全药品库房管理制度。凡是入库的药品都必须经过验收合格才能入库上架;未经验收或验收不合格的药品不得上架～应隔离存放并有明显标识。药品在库房中应该分类上架、摆放齐整、标识正确、先进先出～台账管 理～定期盘存。 内服和外用药品应分类分区存放～并有标识;器械、仪器、棉织品、药品应分库 存放;危险品应在远离火源、水源、电源的指定区域分开存放;过期的药品和器械要 单独存放～明显标识～按规定程序处理。 药品出库时～药品库房保管人员应依据申领凭证～对药品的剂型、规格、数量、 生产厂家和有效期等进行严格的查对后方可出库。 药剂人员应依据本机构执业医师或执业助理医师开具的处方调配发药;药剂人员严格执行查、校、对制度～杜绝差错事故～保证用药安全。 5.计划生育技术服务机构应加强药品使用的监督O定期对库房、药房药品和临床 王二十室急救药品的种类和数量及使用情况进行检查。药品在使用过程中发现质量问题～应该立即停止使用～集中封存～向食品药品监督管理部门申请对同类、同标产品进行 全部复检～根据复检的结果做出处理;不合格产品经发现已用于临床的～要停止使用～并追回服务对象手中的药品～作好处理记录。 6.计划生育技术服务机构需要使用麻醉药品和第一类精神药品的～应当按照《麻 醉药品和精神药品管理办法》取得麻醉药品、第一类精神药品购用印鉴卡O凭印鉴卡 向本省、自治区、直辖市行政区域内的定点批发企业购买麻醉药品和第一类精神药品。要建立麻醉药品和精神药品管理制度～专人负责～专用账册登记～专柜储存麻醉药品 和第一类精神药品。专柜应当使用保险柜～实行双人双锁管理。 执业医师应当使用专用处方开具麻醉药品和精神药品～单张处方的最大用量应当 得合规定～处方的调配人、核对人应当仔细核对～签署姓名～并予以登记;对不符合 规定的～处方的调配人、核对人应当拒绝发药。 7.计划生育技术服务机构因需要使用医疗用毒性药品和放射性药品的～必须按照 圈务院《医疗用毒性药品管理办法》、《放射性药品管理办法》～加强医疗用毒性药品 和放射性药品管理。 9.计划生育技术服务机构避孕药具管理应坚持免费发放、满足需要、减少 浪费、 方便群众的原则～建立健全避孕药具管理制度和避孕药具人员岗位责任制～做好避孕 药具计划编制、药具调拨、储存、发放、使用和不良反应监测等管理。 10.计划生育技术服务机构应加强避孕药具入库及存储管理。依据药具合格证、 标签、说明书、调拨单等对人库避孕药具进行入库验收。对质量不符合规定的药具～ 不准入库。 避孕药具储存应做到安全存储～保证质量～降低损耗～收发迅速～账物相符。人 库避孕药具要按乳胶制品、化学药品的特性和品种、剂型、批号、有效期对药品与非 药品(特别是乳胶制品)、内服避孕药与外用避孕药分类存放整齐、标志明显O避孕 药具出库遵守"先进先出"的原则。 11.凡出现变色、老化、斑点、粘连、潮解、溶化、沉淀等失效不合格避孕药具 不得向下级机构和服务对象发放。对报损、报废药具～必须严格按照国家《不合格药 具处理规定》进行处理。 12.计划生育技术服务机构应做好药物性废弃物的处理。各类过期、变质、失效、 压碎或被淘汰的药品～均应按照《医疗废物管理条例》及相关规定进行有效处理。 第四章 服务过程管理 (一)接诊管理 1.计划生育技术服务机构实行首诊医师负责制～接诊的技术服务人员应根据服务 对象的主诉～对其进行全面的体格检查及必要的辅助检查～做出初步诊断和治疗处理 方案;对需要做检验、检查项目的～应向服务对象交代清楚。对落实避孕措施者～应 向服务对象详细介绍常用避孕方法的作用机理和特点～指导服务对象自主地、负责任 地选择适宜的避孕方法;对需要做特殊治疗、特殊检查或门诊手术治疗的～由接诊技 术人员按常规进行O 2.于术或特殊治疗、特殊检查时～技术人员应严格掌握适应证和禁忌证～在做好 禁忌证排查的前提下～必须征得服务对象的同意～按常规进行检查、治疗或手术;接 诊技术服务人员发现传染性疾病时应按照《传染病防治法》及相关法规的规定～认真 填写传染病报告卡并及时向有关部门报送;对需要转诊的服务对象应及时转诊到上级 计划生育技术服务机构或医疗机构治疗～并为其开具转诊证明～认真书写医疗文书; 对需要住站治疗者～由接诊技术人员开具住站通知单并办理住站于续。 3.计划生育技术服务机构应配备经验丰富、具有亲和力、熟练掌握咨询技巧的技 术服务人员从事咨询工作从事咨询的技术服务人员应认真接待咨询者～正确解答咨询的问题～提出恰当的处理意见。对咨询内容涉及服务对象个人隐私的～技术服务人 员应为其提供单独的咨询服务～并对咨询内容严格保密。 (二)住站管理 1.计划生育技术服务机构应按照入站程序做好服务对象的住站管理。服务对象持 《住站通知单》来计划生育技术服务机构住站～值班护士应协助住站的服务对象办理 人站手续～向其介绍住站须知和注意事项～测量体温、脉搏、呼吸、血压、身高、体 重等常规检查～填写一览表、床头牌～立即通知值班医生处理;值班医生接到通知后 的30分钟内应进行首次查房～做出初步诊断～下达医嘱并分别在8小时、24小时内 完成首次诊疗记录和人站志记录;每天均应查 房～观察住站服务对象情况变化、治疗 处理效果和下达医嘱:并按规定书写每日诊疗记录。 2.计划生育技术服务机构应加强住站管理。计划生育技术服务机构实行值班医生 24小时负责制、集体查房制、床旁交接班制和业务站长或高级职称医师定期查房制; 对住站的服务对象尽快做出准确诊断～制订出最佳的诊疗方案;临床医生和护理人员 应通过观察住站者情况变化和治疗效果及心理状况～分析临床征象.及时调整治疗方 案;临床医生在查房、查体、治疗、服务等活动中～行为、语言、仪态应规范;医嘱、 诊疗活动应按照规范完整记录。 计划生育技术服务机构住站管理中的手术、药品、护士站、康复室等的管理按照 有关规范执行。 3.计划生育技术服务机构应加强出站管理。计划生育技术服务人员应根据住站服务对象的治疗情况确定出站～并向服务对象交代出站后的注意事项以及随访时间;确定住站服务对象出站的～应及时办理出站相关于续。住站服务对象自己要求出站时～ 技术服务人员应向其讲明后果～若仍然坚持出站应让服务对象或其家属在出站记录上签字。 4.计划生育技术服务机构应加强护理管理。计划生育技术服务机构的护理人员要 全面、系统地了解服务对象在心理、生理、精神、生活等方面的整体需求～使服务对象能主动配合诊疗活动;护理工作要做到观察病情严密仔细～资料收 完整～护理诊断正确、措施合理、实施规范;医嘱执行准确、集及时、准确、 及时～护理文件书写正 确、规范、完整～生活护理到位～无护理过错;做好无菌操作、消毒隔离。 5.计划生育技术服务机构应做好病例讨论工作O进行病例讨论是计划生育技术服务人员提高技术水平和服务质量的一个重要途径～应定期或不定期进行计划生育手术 或临床典型病例、疑难病例、手术和死亡病例讨论。 县级以上计划生育技术服务机构应定期或不定期召开由主管科室负责人或副主任 医师以上职称人员主持的临床病例或于术讨论会～主管医师介绍临床典型病例或手术 情况、解答有关病情、诊断、治疗或于术等方面的问题并提出分析意见:临床病例或 手术讨论会应有记录～归纳整理后归入医疗文书内。 凡遇疑难病例～可根据疑难程度进行科室讨论或站内讨论。科室讨论的由科室主 任或主治医师主持～科室有关人员参加;站内讨论的由业务站长主持～技术骨干参加O 通过讨论～明确诊断～提出治疗方案。 对重大、疑难及新开展的于术～必须进行术前讨论。由科室主任或主治医师主持～ 手术医师、麻醉医师、护士长、护士及有关人员参加O制定手术方案和手术风险防范 预案、术后观察事项及护理要求等讨论情况记入医疗文书。 在计划生育技术服务机构死亡的病例～必须进行死亡病例讨论～一般应在死后一 周内进行～特殊病例应及时讨论死亡病例讨论由业务站长主持～技术服务人员参加～ 分析原因～总结教训～提出防范措施。 (三)手术、会诊、转诊和抢救预案 1.计划生育技术服务机构应加强手术管理。接诊的技术服务人员要按照相关法规 和《常用计划生育手术常规》等临床技术规范规定的程序～认真询问和了解服务对象 的身体状况、现病史、既往史、个人史、家族史、婚育史、避孕史等;根据受术者的 健康状况、劳动强度及其所处的生理期～向受术者详细介绍手术的适应证、禁忌证、于术方法和过程、可能的并发症、注意事项～并与受术 者签订知情同意书O 技术服务人员应对受术者进行全面的体格检查及必要的辅助检查～根据检查结果 排除手术的禁忌证。严禁进行选择性别的人工终止妊娠手术、 技术服务人员在实施计划生育手术中～应严格执行《常用计划生育技术常规》、《计划生育技术服务临床技术操作规范一一计划生育学分册》 ～及时完成手术记录。 手术室护士在术前应做好手术室消毒和手术所用的手术包和术中常用的物品、药 口和急救设备和器械的准备和核对;在术中应根据手术需要严格按照《常用计划生育 二术常规》做好器械传递或巡回护理等相关配合工作～术后负责手术器械的清洗、打包、敷料药物的补充～清洁手术室并进行消毒。 2.计划生育技术'服务机构应建立和严格执行知情同意制度。对实行避孕节育手的对象～技术服务人员进行术前谈话～详细说明手术中及手术后可能发生的情况～并 请服务对象签署手术知情同意书;手术中需要麻醉的～麻醉师应了解受术者情况～与 受术者进行麻醉前谈话～详细说明手术中麻醉的目的及麻醉中可能发生的情况～与受 术者签订麻醉协议同意书并按照麻醉规范实施麻醉～对可能致敏的麻醉药物必须按规 定作过敏试验。 3.技术服务机构应建立健全会诊制度。对本科室内较疑难叫生育手 术或病例～由主管医师提出～科室主任召集本科室技术服务人员参加～进行科室内 讨～明确诊断和统一治疗意。 对科室内讨论不能处理的疑难的计划生育手术或疑难病例～需多科室会诊的～由科室主任提出～业务站长同意～邀请有关技术骨干进行站内会诊。站内会诊由业务站长主持～申请会诊科室主治医师提供病史、体检、辅助检查情况以及初步诊断、会诊 目的与要求～经主管医师或科主任报告病情～要作好详细、准确、完整的会诊记录～通过会诊确定治疗方案 本站不能解决的疑难病例～可聘请站外专家来站会诊。由科室主任提出申请～业务站长批准～确定会诊时间～会诊由业务站长主持～站内相关科室的技术服务人员参加～主治医师报告病情经治医师作会诊记录～及时认真执行会诊确定的诊疗方案。 下级计划生育技术服务机构或外站(院)邀请本站会诊者～根据申请会诊机构的要求～派学有专长、临床经验丰富、责任心强的技术骨干外出会诊O会诊时要严肃认真～耐心听取病情汇报～细致地检查服务对象～科学地提出诊疗意见～供下级计划生育技术服务机构或邀请会诊医疗机构参考。 4.计划生育技术服务机构应加强转诊管理。计划生育技术服务机构对入站服务对象中难度比较大的避孕节育手术或不能诊治或治疗预后不良的疾病～由科室主任提出～经主管业务的站长批准～提前与转入机构联系～征得同意后方可转出对在转诊 途中可能加重病情或死亡者～应留站处置～待病情稳定后～再行转诊。较重服务对象 导诊时应派医护人员护送～将病历摘要随病员转去。 5.计划生育技术服务机构应建立各种技术意外的抢救预案。特别是对药物过敏、 麻醉意外、各种休克、DIC (弥漫性血管内凝血)、各种于术并发症、羊水栓塞等～应 建立详细的防范和抢救方案～技术服务人员应熟悉并掌握抢救程序和抢救预案。 (四)无菌技术管理 1.计划生育技术服务机构应加强站内感染管理～严格控制站内感染的发生O建立感染控制和灭菌管理组织(或感染管理委员会) ～负责站内感染控制制度、消毒灭菌 晋理制度的制定和组织实施～落实感染控制职责～协调站内感染管理方面的重大事项。 县级以上计划生育技术服务机构应设立专(兼)职感染控制员。 2.计划生育技术服务机构应严格执行预防和控制感染管理的各项规章制度～熟悉 站内感染诊断标准;掌握抗感染药物合理应用原则～做到使用合理;发现站内感染案 咧、危险因素、病原菌及其耐药性等问题时～应及时查找感染源、感染途径～控制蔓 毡～积极进行治疗～如实报告;发现有站内感染流行趋势时～及时报告站内感染控制 领导小组～并协助调查;发现法定传染病～按《传染病防治法》及相关法规的规定报告并及时处理。 3.计划生育技术服务机构应严格遵守消毒灭菌原则～凡进人人体组织或无菌器官的医疗用品必须灭菌;凡接触服务对象皮肤黏膜的器具和用品必须消毒O根据物品的 材质和性能选用物理或化学方法进行消毒灭菌O消毒首选物理方法～不能用物理方法 消毒的方选化学方法。耐热、耐f显物品灭菌首选物理灭菌法;于术器具及物品、各种 穿刺针、注射器等首选压力蒸汽灭菌;油、粉、膏等首选干热灭菌;不耐热物品如各 种导管、精密仪器、人工移植物等可选用化学灭菌法～如环氧乙烷灭菌等～内窥镜可选用环氧乙烷灭菌或2%戊二醛浸泡灭菌。 化学灭菌或消毒～可根据不同情况分别选择灭菌、高效、中效、低效消毒剂O使 用化学消毒剂必须了解消毒剂的性能、作用、使用方法、影响灭菌或消毒效果的因素 等～配制时注意有效浓度～并定期监测O更换灭菌剂时～应对用于浸泡灭菌物品的容 器进行灭菌处理。 连续使用的氧气温化瓶、雾化器、呼吸机的管道等器材～必须每日消毒～用毕终 末消毒～干燥保存。湿化液应用无菌水。 4.计划生育技术服务机构应加强对一次性使用医疗用品的管理。统一在符合资质 的经营企业采购～按照订货合同、产品检验合格证、生产日期、消毒或灭菌日期及产 品标识和失效期等～进行质量验收c进口的一次性导管等无菌医疗器械应有灭菌日期 和失效期等中文标识～保管人员要做好收、发、存的记录等日常管理工作。 一次性医疗用品应存放于阴凉干燥、通风良好、距地面?20cm～距墙壁5cm的 物架上。严禁使用包装破损、失效、霉变的一次性医疗用品。使用中若发生热源反应、 感染或其他异常情况～应立即停止使用～留取样本送检～作详细记录并报告质检部门～ 不得自行处理。使用后的一次性医疗用品应按国家的规定进行无害化处理～严禁重复 使用和回流市场。 5.计划生育技术服务机构应做好消毒灭菌重点部位的监控工作。加强对站内感染 的防范和控制～建立健全消毒灭菌管理组织～制订消毒灭菌管理制度～督促相关科室 严格执行消毒灭菌操作规程～降低站内感染率的发生。把子术前施术者子的清洁消毒、 手术室消毒灭菌和感染控制、治疗室感染控制、消毒供应室的感染控制、检验室的感 染控制、诊室、治疗室的感染控制作为重点监控部位～定期开展消毒灭菌效果的检测 工作。 对感染性手术应注明感染情况～严格隔离管理。术后器械及物品双消毒～标本按 隔离要求处理～手术间严格终末消毒;手术废弃物品须置黄色或有明显标识的塑料袋 内～封闭运送～无害化处理。 治疗室内布局合理～清洁区、污染区分区明确～标志清楚;元菌物品按灭菌日期 依次放入专柜～过期重新灭菌～设有流动水洗手设施;无菌物品必须一人一用一灭菌; 抽出的药液、开启的静脉输入用无菌液体须注明时间～超过2小时后不得使用;启封 抽吸的各种溶媒超过24小时不得使用;置于无菌储槽中的灭菌物品(棉球、纱布等) 一经打开～使用时间最长不得超过24小时;治疗车上物品应排放有序～上层为清洁 区～下层为污染区～并且应配有快速消毒剂。治疗室内治疗、护理及换药操作应按 清洁伤口、感染伤口、隔离伤口依次进行～特殊感染伤口应就地严格隔离～处置后进行严格终末消毒～不得进入治疗室;感染性敷料应放在黄色防渗漏的污物袋内～及时 焚烧处理。 严格按照卫生部《消毒供应室验收标准》～做好消毒供应室的感染控制O压力蒸汽灭菌操作程序按《医院消毒技术规范》。灭菌合格物品应有明显的灭菌标志和日期～专室专柜存放～在有效期内使用;一次性使用无菌医疗用品～拆除外包装后～方可移入无菌物品存放间。对购进的原材料、消毒洗涤剂、试剂、设备等进行质量监督～杜 绝不合格产品进入消毒供应室。 做好检验室的感染控制～严格执行无菌技术操作规程～静脉采血必须一人一针一 管一巾一带;微量采血应做到一人一针一管一片;对每位服务对象操作前要洗手或进行于消毒;使用合格的一次性检验用品～用后进行无害化处理;无菌物品如棉签、棉球、纱布等及其容器应在有效期内使用～开启后使用时间不得超过24小时。使用后的 废弃物品～应及时进行无害化处理～不得随意丢弃; 各种废弃标本应通过焚烧、消毒或灭菌、入污水池各种器具应及时消毒、清洗; 的方式进行分类处理;进行各种检验时应避免污染～对特殊传染性服务对象检验后～应及时进行消毒～遇有场地、工作服或体表污染时～应立即处理～防止扩散。 做好分娩室的感染控制。分娩室周围环境必须清洁、无污染源～布局合理～严格 区分无菌区、清洁区、污染区～区域之间标志明确;墙壁、天花板、地面光滑～有良 子gj排水系统。对患有或疑似传染病的服务对象应隔离待产、分娩～按隔离技术规程 二理和助产～用后的一次性用品及胎盘必须放入黄色塑料袋内～密闭运送～无害化处理～.对房间严格进行终末消毒处理。患有皮肤化服及其他传染性疾病的工作人员～应严格控制进入。 做好诊室、治疗室的感染控制室内布局合理～清洁区、污染区标志清楚～严格 主用一次性消毒物品～保证一人一器一垫～定期更换室内消毒物品～每日用消毒液擦拭室内物品～湿式拖把拖地～每周进行一次紫外线消毒。 医疗废物贮存设施、设备应当定期清洁和消毒。 第五章文书档案管理 (一)医疗文书管理 1.计划生育技术服务机构应加强医疗文书管理。县级以上计划生育技术服务机构 应该建立接诊(门诊)记录制度～对服务对象在计划生育技术服务机构接受的各种服务都应做好记录。 计划生育技术服务机构的技术服务记录包括接诊服务记录、手术记录、住站记录、 孕前服务及生殖健康服务记录、咨询记录、出诊记录、培训指导记录、随访记录、避 孕药具发放记录等。 住站记录包括:入站记录(志)、病程记录、上级医师查房记录、病例讨论记录、 交(接)班记录、会诊、转诊记录、于术记录、辅助检查记录、护理记录、 出院记 录等。 2.计划生育技术服务记录应按照《病历书写基本规范》的要求和《计划生育技 术服务医疗文书书写式样》的内容和格式书写工整、清晰～诊断准确～语言通顺～表 达规范～标点正确～做到客观、真实、准确、及时、完整。 3.计划生育技术服务机构应加强首次服务或病程记录的管理。首次服务或病程记录应当在服务对象人站8小时内完成。因抢救急危服务对象～未能及时书写的～应在 抢救结束后6小时内补记～并加以注明主治医师首次查房记录应在服务对象人站的 48小时内完成O转出记录由转出科室医师在服务对象转出科室前书写完成(紧急情况 除外) ;转入记录由转入科室医师于服务对象转入后的24小时内完成。手术记录应当 在术后24小时内完成手术护理记录应当在手术结束后及时完成。出站记录应当在服务对象出站后的24小时内完成。抢救记录的抢救时间应当具体到分钟。死亡记录应当 在服务对象死亡后24小时内完成。计划生育技术服务文书书写过程中出现错误～应当 用双线划在错字上～在错字的上方重写～不得采用刮、粘、涂等方法掩盖或去除原来 的字迹。 4.计划生育技术服务机构应加强处方管理。严格按照《处方管理办法(试行?、《药事管理暂行规定》～建立健全处方和处方用药管理制度～加强处方开具、调剂、使用、保存的规范化管理～促进合理用药～保障服务对象的用药安全。 处方的调剂应当遵循安全、有效、经济的原则;技术服务人员应当根据医疗和服务的需要～按照新版《中国药典》、诊疗规范、药品说明书中的药品适应证、药理作 用、用法、用量、禁忌证、不良反应和注意事项等开具处方。开具麻醉药品、精神药 品、医疗用毒性药品、放射性药品的处方须严格遵守有关法律、法规和规章的规定。 处方格式、书写、审核、调剂和保管应符合《处方管理办法(试行?的规定O 处方由调剂、出售处方药品的部门或计划生育技术服务机构的药房妥善保存。普 通处方、急诊处方、儿科处方保存1年～医疗用毒性药品、精神药品及戒毒药品处方 保存2年～麻醉药品处方保存3年。处方保存期满后～经计划生育技术服务机构负责 人或者药事委员会批准、登记备案～方可销毁。 (二)资料、档案管理 1.计划生育技术服务机构应加强资料、档案的管理。建立健全技术服务资料档案 管理制度～落实资料档案管理人员～明确资料档案管理岗位职责～对资料档案的形成、 保管、使用进行有效的控制。 2.计划生育技术服务机构应加强档案室的管理。建立档案接收记录～按立卷标 准～及时分类整理、立卷、编目O档案室管理人员应对住站医疗文书、手术记录等重要记录进行定期检查和核对。建立并执行档案保密及借阅制度～住站医疗文书资料的借阅、复印或复制应严格手续～控制范围～保存必要的申请、法定证明和有效身份 证明。 定期对档案进行鉴定～对保存期限己满的档案报请上级批准后进行销毁～对档案保管不善造成毁坏和丢失～应按规定追究有关人员责任。 第六章环境安全管理 1.计划生育技术服务应加强标识管理。在服务场所、办公场所设置必须的信息标识、导向标识和警示标识。在服务对象相对集中或人员流动比较大的地 方设置共信息标识;在计划生育技术服务机构内设置平面布局示意图、服务时间、服务项目、主要技术服务人员等标识。 在各科室相应位置设置防火、严禁吸烟、讲究卫生、紧急出口、防触电等警示标 lR;影像科室有禁止进行非医学需要的胎儿性别鉴定的标识;特殊科室、医疗废物处 理和医疗废物专用包装物、容器等应有特别警示标识;服务区内的公共场所应设置禁止吸烟、不随地吐痰、不乱丢纸屑的标识。各种标识制作应统一规范～位置显著得体～ 悬挂安全牢固。 2.计划生育技术服务机构应加强人员标识管理。技术服务人员在工作期间应佩带 有姓名、职务或职称、编号、本人照片等内容的胸牌～技术服务人员编号应保持本机 句的惟一性。 3.计划生育技术服务机构要设置服务状态标识。护士办公室应摆放"住站一览表"～内容包括:科别、姓名、住站号、床号、人站时间、诊断、护理级别、危重符 号等;住站者所住床位应设有床头牌～床头牌应注明:科室、姓名、性别、年龄、入 站时间、饮食标识、过敏标识、护理级别等;于术室在进行手术时门口应有"手术 中"标识;X线机房门外要有电离辐射标志～在进行透视或X光片拍摄时～应有醒目 的工作指示灯;
