孕产妇保健知识宣教 妇幼保健知识 一、孕产期保健服务 孕产期保健服务是指从怀孕开始至产后42天为孕产妇、胎儿及新生儿提供的系统检查、监护和保健指导，包括孕产期卫生指导、基本的孕产妇系统保健、胎儿保健、新生儿保健、孕产期卫生咨询和疾病防治。孕产妇系统保健管理应以提高产科质量为中心，筛选高危孕妇为重点。 (一)孕产期卫生指导 孕产期卫生指导是指对准备怀孕的育龄妇女、孕产妇及其丈夫进行生殖保健知识的宣传教育。在产褥期随访时，为每个随访对象进行指导，进一步强化产褥期保健知识。 孕产期卫生指导内容如下: 1、生育时间的选择(5、6月份怀孕，3、4月份分娩较好)。 2、孕产期合理营养(用药选择、避免接触有害物质)。 3、孕产期心理卫生。 4、孕妇自我监护知识(胎动计数、听胎心、称体重、测宫高)。 、孕妇需及时就诊的危险症状(阴道流血、严重的头痛、严重的呕吐、高烧)。 5 6、分娩前准备和分娩配合。 7、母乳喂养。 8、新生儿护理及科学育儿知识。 (二)基本孕产期系统保健 1、孕期保健 孕期保健应从孕前三个月开始，并且越早越好。由于各期都有应做的检查、宣教及处理的特殊内容，因此各期保健均不可忽视。 现将孕期保健内容分述于下。 (1)孕早期保健 ——指孕12周内 ?及时识别早孕症状，及早开始保健，早孕期至少检查一次。以排除异常情况，如宫外孕，葡萄胎。 ?建立和填写孕产妇保健卡、册 A.详细询问病史:一般情况、本次怀孕情况、既往妊娠史、既往病史、月经史、家庭史、遗传病史等。 B.全身检查:测基础血压、体重、身高、心、肺、肝、脾、甲状腺、下肢有无水肿、乳房发育有无异常。 ?识别高危，确定能否继续妊娠。早孕检查应在停经?12周内进行，高危妊娠的识别、筛查、管理、按照高危及识别要点、筛查方法及处理原则进行。 ?早孕指导:避免一切不良因素的影响，如病毒、药物、放射线等。 (2)孕中期保健 ——指孕13—28周 ?在孕20—24周至少检查一次，高危妊娠应增加检查的次数。 construction unit to ensure a quality installation, first to pre-installed pivot, measured the door blade and the arm connecting plate ... Temporary will water seal plate pressure in sealed article Shang, in level direction and vertical direction check water seal of straight degrees whether meet requirements, Hou with Mark pen from gate Panel of check water bolt hole within draw bolt hole in check water article Shang of location, in corresponding of mark Shang drill out sealed article of bolt hole, with bolt try dressed Hou, removed has joint of water seal, with strong hot rubber of way, will water seal firmly stick received in with, forming Hou with wheel machine gently processing rugged of surface, and check water seal round head surface uneven degrees, each across 0.5M measuring is, After passing on (3)孕晚期保健 ——指孕28—40周 孕28周后争取每2—4周检查一次，至少检查3次。如属高危应当增加检查次数。 (4)孕期保健指导 注意休息，避免重体力劳动，预防早产。加强营养，必要时补充营养物质如钙、铁等。指导乳房护理，宣传母乳喂养的好处，做好临产前的准备，介绍临产征兆。产前检查总次数不应少与5次。 产后产妇访视 ?产后常规访视3次，时间分别为出院后三天内，第14天、28天，有异常情况酌情增加访视次数。 ?填写《孕产妇保健手册》，了解产前、产时、产后母婴状况。 ?了解一般情况:精神、睡眠、饮食、大小便、心理状态等。 ?测体温、脉搏，必要时测血压。 ?检查乳头有无皲裂，乳房有无红肿、硬结、乳汁分泌量，乳腺管是否通畅。 ?了解母乳喂养情况，进行指导与咨询。 产后访视指导 1. 保证吃好、休息好:由于分娩会给产妇的身心造成极度劳累，所以分娩后的第一件事就是让产妇美美地睡一觉，家属不要轻易去打扰她。睡足之后，应吃些营养高且易消化的食物，同时要多喝水。“月子”里和哺乳期都应吃高营养、高热量、易消化的食物，以促使身体迅速恢复及保证乳量充足。 2. 尽早下床活动:一般情况下，经阴道正常分娩的产妇在产生第二天就应当下床走动，这不仅有利于产后体力恢复、增加食欲，也有助于子宫收缩，促进恶露的排出及子宫复原。但应注意不要受凉并避免冷风直吹。也可以在医护人员指导下，每天做一些简单的锻炼或产后体操，有利于恢复，并保持良好的体形。产生1个星期，产妇可以做些轻微的家务活，如抹抹桌子、扫扫地等，但持续时间不宜过长。主要目的是活动身体，不应产生疲劳感，更不可干较重的体力活，否则易诱发子宫出血及子宫脱垂。 3. 特别注意个人卫生:“月子”里产妇的会阴部分泌物较多，每天应用温开水或1:5000高锰酸钾溶液清洗外阴部。勤换会阴垫并保持会阴部清洁和干燥。产生由于出汗多，要经常洗头、洗脚、勤换内衣裤，保持体肤的清洁。洗澡以淋浴为宜，以免脏水流入阴道内发生感染。产妇“坐月子”期间，吃的东西较多，吃的次数也较多，如不注意濑口刷牙，容易使口腔内细菌繁殖，发生口腔疾病。过去，有不少妇女盲目信奉“老规矩”--坐月子里不能刷牙，结果“坐”一次“月子”毁了一口牙。产妇每天应刷牙一两次，可选用软毛牙刷轻柔地刷动。每次吃过东西后，应当用温开水濑濑口，居室内经常通风，室内温度不可太高，也不可忽高忽低，将门窗紧闭，不论何时产妇都要盖厚被的传统做法是十分危险、十分有害的，尤其是在夏季，极易造成产妇中暑。 4. 尽早哺乳:分娩后乳房充血膨胀明显，尽早哺乳有利于刺激乳汗的分泌，使以后的母乳喂养有个良好的开端;还要促进子宫收缩、复原。哺乳前后，产妇十分注意保持双手的清洁以及乳头、乳房的清洁卫生，防止发生乳腺感染和新生儿肠道感染。 5. 合理安排性生活:恶露未干净或产后42天以内，由于子宫内的创面尚未完全修复，所以要绝对禁止性生活。因为很容易造成产褥期感染，甚至造成慢性盆construction unit to ensure a quality installation, first to pre-installed pivot, measured the door blade and the arm connecting plate ... Temporary will water seal plate pressure in sealed article Shang, in level direction and vertical direction check water seal of straight degrees whether meet requirements, Hou with Mark pen from gate Panel of check water bolt hole within draw bolt hole in check water article Shang of location, in corresponding of mark Shang drill out sealed article of bolt hole, with bolt try dressed Hou, removed has joint of water seal, with strong hot rubber of way, will water seal firmly stick received in with, forming Hou with wheel machine gently processing rugged of surface, and check water seal round head surface uneven degrees, each across 0.5M measuring is, After passing on 尿布用柔软吸水性好的棉织品，做到勤洗勤换，通常白天要换4次以上，晚上应换2次以上，每次更换时均应清洗小屁股，并外涂适量护肤油剂;尿不湿则选择质量较好且透气性能好的，在家里时尽量用尿片，出门或睡觉时则用“尿不湿;注意尿片或尿不湿包裹不宜太紧，以便四肢自由伸展。 3.睡眠和睡姿 睡姿影响呼吸，且新生儿头颅比较软，良好的睡姿有利于头颅的发育。 最好的睡姿是仰卧或侧卧，以避免压迫胸肺部，建议在喂养后多采取侧睡，以免溢奶或呛咳造成窒息;在采取仰卧位时，应当经常变换体位;足月儿因活动力较强，出生头几天可以适当采取俯卧，以利呼吸道分泌物流出，防止呕吐物倒流入气管，但俯卧必须拿去枕头，头侧向一面，此时要有家长在一旁监护。 4.哺乳和喂养 母乳喂养时应采取“竖抱位”即头部略抬起，这是最理想、最符合自然规律的喂奶方式。 人工喂养尽量不要直接喂服新鲜奶，因为其中所含的蛋白质等营养成分不适合新生儿;混合喂养(母乳喂养和代乳品喂养相结合)时，应先以母乳喂养为主。 人工喂养时奶嘴洞大小应适中并注意温度，奶嘴喂奶时尽量不要让宝宝吸进空气，以免吐奶，而喂完之后可轻拍宝宝背部，以免积气。此外要对奶瓶、奶嘴严格煮沸消毒。 喂养不需太讲究定时，一般情况下3小时左右喂一次，每次以吃饱吃好为原则:即宝宝吃奶后不哭不吵，且体重正常增长。 5.预防感染 护理新生儿时，要注意卫生，在每次护理前均应洗手，以防手上沾污的细菌带到新生儿细嫩的皮肤上面发生感染，如护理人员患有传染性疾病或带菌者则不能接触新生儿，以防新生儿受染。如新生儿发生传染病时，必须严格隔离治疗，接触者隔离观察。产母休息室在哺乳时间应禁止探视，以减少新生儿受感染的机会。 6.皮肤护理 出生不久的新生儿，在脐带未脱落前，尽量不用盆浴，而采用干洗法为新生儿擦身。脐带脱落后，则可给予盆浴，宜用无刺激性的婴儿专用香皂，浴后要用干软的毛巾将身上的水吸于，并可在皮肤皱褶处涂少许香粉。每次换尿布后一定要用温热毛巾将臀部擦干净，有时因尿液刺激使臀部皮肤发红，这时可涂少许无菌植物油。寒冷季节臀红明显时，还可用电吹风在红臀局部吹烤，每日3,4次，每次5,10分钟(电吹风不可离皮肤太近，以防烫伤)。 7.五官护理 应注意面部及外耳道口、鼻孔等处的清洁，但勿挖外耳道及鼻腔。由于口腔粘膜细嫩、血管丰富，极易擦伤而引起感染，故不可经常用劲擦洗口腔，更不可用针、特别是不洁针去挑碾牙龈上的小白点——上皮小珠(俗称“马牙”或“板牙”)，以防细菌由此处进人体内而引起败血症。 新生儿护理要点 新生儿的护理需要特别细致周到，护理新生儿初步要注意以下要点: 1.清理口腔:胎儿娩出时应迅速清除口咽部的粘液和羊水，以免误吸，引起吸入性肺炎，但不要擦洗口腔，因新生儿口腔粘膜薄嫩，易受损伤。如果出现“鹅口疮”?—口腔粘膜出现点片状的白膜，可轻轻涂擦制霉菌素药水。 2.保温:新生儿出生后应立即将其全身轻轻擦干，用洁净温暖的棉毯包裹。construction unit to ensure a quality installation, first to pre-installed pivot, measured the door blade and the arm connecting plate ... Temporary will water seal plate pressure in sealed article Shang, in level direction and vertical direction check water seal of straight degrees whether meet requirements, Hou with Mark pen from gate Panel of check water bolt hole within draw bolt hole in check water article Shang of location, in corresponding of mark Shang drill out sealed article of bolt hole, with bolt try dressed Hou, removed has joint of water seal, with strong hot rubber of way, will water seal firmly stick received in with, forming Hou with wheel machine gently processing rugged of surface, and check water seal round head surface uneven degrees, each across 0.5M measuring is, After passing on 室温不能低于23?。新生儿体温应保持在36,37?。生后第一天每4小时测一次体温，体温稳定在36(5?左右时，可改为每6,12小时测一次。若体温低于36?或高于38?时，应查找原因，进行处理。 3.滴眼:初生后即用0(5%新霉素或0(25%氯霉素滴眼，以防新生儿眼炎。眼睛分泌物多时，可用生理盐水或2%硼酸棉球拭净后再滴眼药。 4.体位:除妈妈抱起喂奶外，新生儿整日卧床休息。应保证有足够睡眠时间，每日在20小时以上。最好采取侧卧位，尤其喂奶后应向右侧卧，平时采取左侧卧。经常变换体位，可防止睡偏头。仰卧不安全，此种体位，如漾奶时，可引起窒息。可不必枕枕头，如枕时枕头的高度应大致同肩宽，更不能用硬枕头来矫正头形。 5.注意居住环境:居住环境要特别注意两个因素:第一是通风因素。新生儿的居住环境要求有适当的通风气流，同时要避免传统房屋坐南朝北格局的穿堂风;第二是噪音因素。高分贝、刺耳噪音要注意隔离，以免对宝宝的听觉器官造成伤害。 6.注意冷热护理:因为新生儿体温调节机能差，因此，冬天要保暖，夏天要防暑降温，平时要根据气温的变化及时增减衣服。 7.注意皮肤护理:新生儿皮肤娇嫩，容易损伤，因而接触动作要轻柔，衣着要宽松，质地要柔软，不宜钉扣子或用别针。要用温水擦洗皮肤皱折处，每次大小便后清洗，并用毛巾擦干。 8.注意脐带护理:在新脐带未脱落时，每天用0.5%聚乙烯醇醚络碘溶液擦洗脐部一次，然后用消毒纱布盖上，不要放盆内洗澡。脐带脱落后，可以不用纱布，但必须保持脐部干燥清洁。发现脐部有红或有脓性分泌物，则应进行消炎处理。 9.要保证充足睡眠:经常变换新生儿的睡姿，以防止头颅变形。 10.处理特殊生理现象:如所谓的新生儿“马牙”、女婴出生后数天内阴道有粘液或血性分泌物，红尿、乳房肿大、红斑、色素斑以及生理性黄疸(出生后2-3天出现)等，这些过几天后就会自然消失，不必特殊处理。如果时间较长或有其它不良反映，则应去医院检查。 新生儿护理的误区 1.生理性黄疸误认为肝炎:新生儿在生后2,3天开始出现黄疸，4,5天后最明显，7,14天自然消退，一般情况良好，无不良反应，称“生理性黄疸”。如果10天内黄疸消失，就不是病态，更不是肝炎，叫“生理性黄疸”。它一般在生后第4,5天出现，一周左右消失。早产儿黄疸可持续到14天消失，产生原因: (1)孩子体内红细胞破坏增加，使血中间接胆红素增加，使皮肤发黄。 (2)新生儿肝脏发育不成熟，肝细胞产生的酶(葡萄糖醛酸转移酶)活性不足，不能有效的将间胆转化为直胆而由胆道排泄，间胆在血中浓度增高，引起皮肤黄染。总之，生理性黄疸属正常生理过程，不需要治疗。 2.挤压乳腺:新生儿的乳房在生后第4,5天出现轻度肿胀，并有少许乳汁溢出，7,10天达高潮。这是因为母亲在妊娠后期体内分泌雌激素(孕激素及催乳素)，致使胎儿通过胎盘吸收了较多的激素所造成的乳腺一时性肿胀，无论男孩、女孩都可有，属于生理现象，2,3周即可消失，千万不要挤压，它的恶果就是患“乳腺炎”。 3.错弃初乳:初乳是指产后12小时以内分泌的乳汁，因初乳颜色太黄，比较清淡，所以有的人认为“初乳”是“坏乳”而白白挤掉，甚为可惜。因为初乳营construction unit to ensure a quality installation, first to pre-installed pivot, measured the door blade and the arm connecting plate ...
