预防接种工作 一、为保证这项工作经常性开展，乡(镇)防保所设立以所长为组长，乡(镇)防疫人员及接种员人员为骨干的计划免疫领导组，负责制订本乡(镇)生物制品计划，领发生物制品，实施预防接种，调查处理异常反应，“冷链”管理，以及对基层卫生人员计划免疫工作的业务培训、督促、检查等项工作。 二、乡(镇)计划免疫工作实行一卡(儿童免疫接种卡)、一证(儿童预防接种证)、两册(1、2类疫苗接种登记册、新生儿乙肝疫苗接种登记册)、两簿(生物制品令发登记簿、冷链设备接种器材领发登记簿)、两(年度生物制品计划双月报表、0—7岁儿童分村分龄统计表)制度。卡、簿、表由乡防疫人员专柜保管，接种卡仅接种期间发至各接种点，接种完后仍交回乡卫生院管理。7岁后，接种卡片由乡镇卫生院保管15年。儿童预防接种证交儿童家长保管，0—7岁儿童年龄分组表每年统计一次并上墙公布。 三、建、管、用好0—7岁儿童免疫接种卡、证。2003年度建卡率、建证率不得低于99%，卡、证填写符合率不得低于95%以上，坚持每季全乡儿童出生人数与建卡数、建证数核对。 四、接种人员应以高度责任心，严格的科学态度，掌握免疫程序，生物制品性质，接种方法、接种途径及接种前的询问和告知，禁忌症，以及接种后副反应的观察处理。为确保工作质量，接种前要仔细检查发现禁忌症，严禁使用过期、冻接和变质的疫(菌)苗，实行一人一针一管，严格无菌操作。 五、做好接种前的调查摸底工作，接种中的组织实施;填写好接种卡证和接种登记册;接种后的统计汇总工作，按时向县疾控中心报出生物制品使用双月报表或工作总结。 六、积极处理预防接种中出现的异常反应，并及时向县卫生局、县食品、药品监督局报告，必要时，提请县异常反应诊断小组会诊，任何医疗单位或个人不得单方面出具诊断证明，所出证明一律无效。 预防接种安全注射制度 对疫(菌)苗应用灭菌的注射器和规范的操作进行注射，并对使用过的注射器具进行安全处理，称为安全注射，安全注射必须达到对接受注射者无害，对实施注射者无危险，注射后的物品(废物)不会给公共带来危害。根据以上要求，特制定安全注射制度。 一、预防接种要使用合格的注射器(包括注射器要无菌包装，在有效期内使用;接种前才能打开包装，使用后放入指定的安全盒或防剌容器中，不允许再次使用); 二、实施预防接种的人员要持技术合格证、上岗证上岗。 三、预防接种必须掌握各种疫(菌)苗的禁忌症。 四、预防接种的操作要规范化。 五、预防接种的环境要符合工作要求。 六、接种后的接种器材及其废弃物品安全的回收、销毁。 工作制度 一、预防接种门诊每周定若干天数为预防接种日，为辖区内适龄儿童与流动人口儿童开展 常年免疫接种和业务咨询。预防接种门诊要设 立登记咨询点，接受群众咨询。 二、严格按照卫生部颁发的《计划免疫技术管理规程》和《安徽省预防接种免疫程序》its/units: 160 hours 4, major: automobile maintenance and testing specialty second nature and task 1, course, course: this course is a professional course of automobile maintenance and testing specialty. 2, task: the teaching of this module is to the automotive air conditioning repair class teaching in conjunction with training courses, small-class teaching and practical training in the teaching process, theory for sufficient degree, with a focus on fostering students ' practical skills. (A) basic knowledge teaching target understand car air conditioning air regulation of based knowledge understand car air conditioning of structure principle understand car air conditioning of classification and layout understand car air conditioning of cla 要求，做好预防接种实施。 三、本地户籍儿童出生后1个月，外来儿童寄居3个月以上，建立预防接种册、证。预防接种册由接种单位保管，预防接种证必须由儿童家长或监护人保管，入托、入园儿童可由托幼机构暂时代管。儿童居住地变动时要及时办理入册或注消手续。儿童入托、入园、入学实行查验接种证制度，未按规定程序进行接种的儿童必须到所属接种门诊补种后方可入学。 四、预防接种门诊工作人员应具备工作责任心，取得执业或助理执业医师(护师)资格，并经过县级以上计划免疫知识技术培训合格后方能上岗。上岗工作应佩带胸卡。接种时要穿戴工作衣帽、口罩，佩戴胸卡，患手部皮肤病或传染病期间不准参加接种工作。接种人员应主动向群众宣传计划免疫知识，预约下次接种时间。尚未完成基础免疫且连续通知两次均未前来接种的儿童，及时进行随访落实。 五、保持预防接种门诊清洁卫生，开诊前后要用合格浓度的消毒液擦拭消毒工作台与地面，开启紫外线灯消毒室内空气。每次消毒应做好记录备查。接种前做好准备工作，包括统计应种对象、发接种通知，准备疫苗、注射器及各种药械等。 接种医师岗位职责 1、热爱本职工作、事业心强、熟练掌握本专业知识，在计免医生指导下工作。 2、按规范要求完成各种疫苗的接种操作，协助处理预防接种异常反应。 3、填发接种通知单。 4、逐日、详细、准确地做好门诊日志记录。 5、做好接种前器材的准备，正确使用保养冷链设备和接种器材，按时进行冷链温度监测。 6、负责疫苗的管理及疫苗帐册的建立与使用。 7、担任接种门诊的卫生保洁工作。 8、协助进行接种后回访。 疫苗 一、疫苗使用要有计划性，根据每月接种日安排，准确合理地制定用苗计划。 二、疫苗使用应遵循“足量、适量”的原则，既不能紧缺也不能浪费。 三、疫苗领发手续要完备，使用要有详细登记，包括名称、规格、批号、效期、产地、领苗日期及数量等，帐目要清楚，帐物要相符。 四、疫苗领用一般每月一次，接种门诊用苗有剩时，如冷藏条件具备，在效期内转下次使用。 五、疫苗使用应严格执行有关规定，活疫苗开启半小时，灭活疫苗开启1小时，即应废弃。 六、疫苗应按规定的温度贮存和运输。 七、疫苗应由专人管理，按品名、批号效期，分类整齐存放，短效期先用，长效期后用。 八、接种现场要求一苗一冷藏，即一个冷藏包只冷藏一种疫苗及其稀释液。 its/units: 160 hours 4, major: automobile maintenance and testing specialty second nature and task 1, course, course: this course is a professional course of automobile maintenance and testing specialty. 2, task: the teaching of this module is to the automotive air conditioning repair class teaching in conjunction with training courses, small-class teaching and practical training in the teaching process, theory for sufficient degree, with a focus on fostering students ' practical skills. (A) basic knowledge teaching target understand car air conditioning air regulation of based knowledge understand car air conditioning of structure principle understand car air conditioning of classification and layout understand car air conditioning of cla 疫苗接种注意事项 一、接种前家长应掌握儿童的身体状况，如有明确的过敏史则不能进行疫苗接种;如正在发热或患有各种疾病应暂缓接种;如属早产儿或营养不良、体质较差者，也应暂缓接种。 二、孩子接种后要在接种场地休息15-20分钟，无反应方可离开。接种后要适当休息，不做剧烈运动，不吃刺激性食物，暂时不要洗澡。服用脊髓灰质炎糖丸疫苗后，半小时内不得喂奶或吃热东西。 三、孩子接种后如出现高热或其它接种反应要及时请医生诊治。 四、有些疫苗需按一定的间隔时间连续接种多次才能有效，所以一定要按照规定的免疫程序和日期进行接种。 五、如孩子经常外出，应告知接种医生，办理相关手续。外出时应注意到当地预防保健机构咨询、接种，以免漏种相关的疫苗。 预防接种门诊消毒制度 一、预防接种门诊工作人员必须规范穿戴工作衣帽，勤剪指甲。工作前须用肥皂、流动水洗手，戴好口罩后方可进行接种工作，接种间隙也要勤洗手。 二、保持登记体检室、侯种室、接种室等环境整洁卫生，光线明亮，空气流通。每天下班前须用消毒液对接种台面消毒清洁，接种室和候诊室室内空气消毒用紫外线灯照射30分钟以上(新装紫外线灯管照射强度应?90UW/cm2，使用中的紫外线灯管照射强度应?70UW/cm2，<70UW/cm2的灯管应及时更换)，紫外线灯按每M3空间?1.5瓦安装，紫外线灯管表面应保持洁净，每2,3周用酒精擦拭1次。 三、皮肤消毒液必须密封保存，在有效期内使用，使用中的消毒液须每周更换2次，盛装的容器每周消毒2次。接种部位要避开疤痕、炎症、硬结和皮肤病变处，皮肤消毒时可用无菌棉签浸润2.0%碘酊，涂擦注射部位1遍，作用1分钟，再用75%酒精擦拭2遍，擦净余碘干燥后方可注射;使用0.5%碘伏则直接涂擦皮肤2遍干燥后即可注射。消毒操作以注射部位为中心，由内向外缓慢旋转、逐步涂擦，消毒面积不小于5cm×5cm。局部消毒后注意不得触碰污染。接种活疫苗时不能用碘酊消毒，局部用75%乙醇消毒时，待干后再接种。 四、严格执行无菌操作规程，疫苗接种务必一人一针一管。使用后的一次性注射器必须就地消毒(用1000mg/L含氯消毒剂或2.0%过氧乙酸消毒剂浸泡60分钟)毁形，放入专用收集袋，严禁出售或随意丢弃。因无回收指定单位无法集中处理的，应及时焚烧处理。一次性注射器使用后用毁型机毁型或将针管、针头分开放入有消毒液的防刺破安全盒或回收桶。 五、保持工作台面及其他物体表面的洁净，每个工作日前用300,500mg/L含氯消毒剂或其他消毒剂擦拭消毒。 六、地面采取湿式清扫，用300,500mg/L含氯消毒剂或其他消毒液湿拖地面。 冷链设备管理制度 1、冷链设备必须专人管理，明确职责。 2、冷链设备必须专用于预防接种用生物制品的贮存、运输，不可挪作他用。 3、冷链设备要建卡、建帐、建档、上、下帐目相符，帐物要一致。 its/units: 160 hours 4, major: automobile maintenance and testing specialty second nature and task 1, course, course: this course is a professional course of automobile maintenance and testing specialty. 2, task: the teaching of this module is to the automotive air conditioning repair class teaching in conjunction with training courses, small-class teaching and practical training in the teaching process, theory for sufficient degree, with a focus on fostering students ' practical skills. (A) basic knowledge teaching target understand car air conditioning air regulation of based knowledge understand car air conditioning of structure principle understand car air conditioning of classification and layout understand car air conditioning of cla 4、冰箱必须安装在远离热源、通风良好、避光干燥、无震动、无腐蚀的场所，放置平稳。底部安有10cm的支架，上部和背部要分别留下40.30cm的空间。 5、冰箱要配有专用稳压保险装置，并有统一的标志和编号。 6、冰箱内不能存放私人物品或非生物制品。 7、 贮存疫苗时，存量不能超过设备容积的2/3，且疫苗间要留有间隙，保持空气流通，保证制冷效果。 8、冰箱内应有温度计，每天上班后、下班前两次记录温度，设备发生故障时，应及时报告、检修。 9、冷链设备应定期清理，一般每月一次，保持内外清洁干燥。 10、冷藏包要专架清洁存放，冰排倒空洗净后存放。 疫苗和冷链管理制度 一、疫苗实行一个窗口专人管理，供应渠道严格执行:省—市—县—镇—接种门诊(接种点)。健全疫苗领发保管制度，建立疫苗领发台帐，疫苗的出入账物相符，登记必须有疫苗的名称、数量、生产厂名、批号、失效期、进出数量、结余数量、领取人、备注等。 二、根据现行的免疫程序，本辖区的总人口数，出生率，各年龄组人口数及疫苗的损耗系数等制订疫苗计划，每年三月前将下一年度的计划免疫用苗数量报上级疾病预防控制机构。 三、疫苗的运输、贮存和使用要严格按照有关的温度要求进行。按照疫苗的品种、批号分类整齐码放，疫苗纸箱(盒)之间、与冰箱冰柜壁之间均应留有冷气循环通道。分发使用疫苗按照“先短效期、后长效期”和同批疫苗按“先入库，先出库”的原则，存放要整齐，包装标志明显，疫苗之间留出冷气循环通道。 四、健全冷链设备管理制度，建立冷链设备台帐，记录各种设备的品名、型号、到货时间、数量;建立设备运转与维修记录簿，记录发生故障与维修情况。疫苗过期应及时做好报损手续。 五、冷链设备做到专人管理，定期保养，建立维修、温度记录。冷链设备要有专人保养，经常擦拭保洁，每日2次(上午上班后与下午下班前)观察记录冰箱冰柜内运转温度;保冷背包每次用后及时擦净晾干备用，冰排用后及时送回冷冻室冻存。 六、冷链冰箱和冰柜应安放在干燥、通风、避免阳光直射、远离热源的地方，后部要留有空间，底部要垫搁架，电源线路与插座应专线专用。 七、所有计划免疫冷链设备仅专用于贮存疫苗，任何单位和个人不得挪用，存放疫苗的冰箱和冷库严禁存放其他物品、过期疫苗。 预防接种体检与登记制度 一、核对受种者免疫卡证、姓名、出生年月日，以及既往接种记录，确认是否为本次某种疫苗的接种对象。若发现预约接种的日期有误，应及时更改，并做好解释工作。 二、询问受种者近期健康状况，以及既往疾病史、过敏反应史、接种反应史等。经询问健康无疾患，再检查体温和其他有关体征，确定可否接种。凡患有接种禁忌症的不得注射或暂缓注射相应疫苗，并在卡证上做好记录。 三、执行“接种告知与家长签字”制度，务必告知家长接种何种疫苗，预防何种疾病，接种后的正常反应，可能出现的异常反应和必要的注意事项等。体检与告知后，医生与儿童家长均需签字负责。 四、用钢笔做好免疫卡证接种日期等项目的工整写登记，接种证上还应记录所使用疫its/units: 160 hours 4, major: automobile maintenance and testing specialty second nature and task 1, course, course: this course is a professional course of automobile maintenance and testing specialty. 2, task: the teaching of this module is to the automotive air conditioning repair class teaching in conjunction with training courses, small-class teaching and practical training in the teaching process, theory for sufficient degree, with a focus on fostering students ' practical skills.
