焦虑症治疗药物研究 [关键词]:5-羟色胺再摄取抑制剂;文拉法辛;三环类抗抑郁剂;苯二氮卓类 健康网讯: 摘要: 传统的药物(如苯二氮卓类、三环类药物)有很多不良反应，10多年来焦虑症药物治疗有了实质性变化，研究 证实，SSRI治疗抑郁症和焦虑症是有效和安全的。现在SSRI已成为治疗惊恐障碍的一线药物。同样在广 泛性焦虑症的长期治疗中，文拉法辛、帕罗西汀和某些三环类药物优于苯二氮卓类。作者认为，文拉法辛、帕 罗西汀、曲唑酮、黛力新以及某些三环类药物可考虑作为广泛性焦虑症的一线药物。苯二氮卓类药起效快，能 立即减轻焦虑症状，可短期应用或与SSRI短期联用。 世界变动不安，社会发展迅速，人们普遍感到沉 重的精神"压力"。各种生活事件引发的应激反应 到处存在，很多人出现了程度不同的焦虑。轻者仅感到 内心的不安，明显的焦虑则不但有主观的紧张不安、恐 惧，而且多伴有自主神经功能亢进，诸如胸闷、心慌，出 汗、震颤与睡眠障碍等，常对诱发焦虑的情境有回避行 为，严重的焦虑则出现惊恐，除焦虑的一般症状外，还 出现过度换气症状和濒死感、将晕倒感、将失控感、将 发疯感等令人恐惧的感觉。但医学心理学研究认为短暂 的，适度的焦虑并无任何危害，甚至还有适应性的益处。 因为适度焦虑向人们发出危险或威胁的信号，有助于人 们动员起来，做好准备，未雨绸缪，克服或避开可能的 危险。有利于人类的生存。只有焦虑过于严重， 和妨碍正常社会和环境 刺激不相称，并且焦虑持续过久，引起人们很大的痛苦 生活时，才成为医学问题。因此，并不 是有焦虑的人都需要进行治疗。 焦虑症的临床类型 目前临床上将焦虑症分为两种类型:惊恐障碍和广 泛性焦虑症(诊断标准请参照CCMD,3，中国精神障 碍分类与诊断标准，第三版，2001)。惊恐障碍(Panic disorder，PD)又称间歇发作性焦虑，基本特征是严重焦 虑(惊恐)的反复发作，每月4次以上。发作时出现头 晕、胸闷、窒息感、心动过速，面部和四肢发麻、出汗、 颤抖等症状，多伴有濒死感或将发疯感。患者常误认为 自己患了心脏病，中风先兆，或内分泌疾病。广泛性焦 虑症(GAD)的基本特征是持续性焦虑不安，并有显著 的植物神经症状、肌肉紧张及运动性不安，病程长达6个 月以上。继发于甲亢、高血压，冠心病等的焦虑称为焦 虑综合征，不称为广泛性焦虑症。 焦虑症治疗的一般考虑 已知焦虑是由于人们觉察到威胁或危险而产生的， 因此焦虑的发病机制被认为是心源性的。过度激活的人 脑警报系统和将内外部刺激信息视为"过度危险"的认知 评价是焦虑产生的基础。因此，焦虑症患者历来是心 理治疗优先选择的对象。近年来认知行为治疗对焦虑症 减轻焦虑症状也是合理的。实际上，由于掌握心理治疗技 术的困难和操作的费时。目前临床上对焦虑症仍然普遍 应用药物治疗。有鉴于此，本文对近年来焦虑症药物治疗 的进展结合笔者的临床经验作一概述，以供同道参考。 惊恐障碍(PD)的药物治疗进展 自Klein(1964)证实丙米嗪(imipramine)能阻断惊 恐发作之后，大量对照研究显示丙米嗪、氯米帕明 (clomipramine)对PD有效，但通常剂量达150mg,d以 上，副作用较大，临床使用有所限制。单胺氧化酶抑制 剂(MAOI)苯乙肼(Nardil)对PD也有效，但同样由于 副作用以及和食物，其他执抑郁药的相互作用，使用受 到限制。苯二氮卓类药物(BZD)曾作为一线抗焦虑药， 但研究显示只有阿普唑仑(alprazolam)、氯硝西泮 (clonazepam)对惊恐发作有效。近年来应用BZD治疗 PD已经减少，原因主要在于PD需要长时间治疗，而长 期使用BZD可能产生依赖，以致日后停药成为一个困难 问题。现在常将BZD和抗抑郁药联用，利用BZD起 效快的优点，惊恐发作控制后短期内将BZD逐渐减量，让 抗抑郁药承担长期治疗的任务。近10多年来出现的选择 性5,HT再摄取抑制剂(SSRI)等新药，安全有效，副作 用小，已成为治疗PD的首选药物。 SSRI类药物 帕罗西汀(paroxetine，赛乐特，seroxat)是美国 FDA批准的第一个治疗PD的SSRI。三个国际性多中心 对照研究证实，其对PD的效果优于安慰剂。治疗剂量 常需40mg,d，75.4%的患者惊恐发作消失。我们自 1997年6月至1999年4月用帕罗西汀治疗84例PD患 者。20mg,d，显效66例，占78.56%，好转18例，占 21.44%，少数患者有恶心、厌食、胃不适、无力等副作 用。治疗中大多自行消失。证实了帕罗西汀对PD的良好 疗效。 舍曲林(sertraline，左洛复，Zoloft)也为FDA批 准用于PD治疗，大样本研究显示59.3%的惊恐发作消 失，剂量为50,200mg/d。 氟伏沙明(fluvoxamine，兰释，Luvox)，研究表明 该药为有效的抗惊恐药，150,300mg,d，2,3的惊恐发 作消失。 氯西汀(fkuoxetine，百忧解，Prozac)，也广泛用于 PD的治疗，Michelson等(1998)证实治疗结果优于安 慰剂。临床经验表明，氟西汀用于PD时初剂量10,20mg/d， 易出现抖动不安、激动、失眠等不良反应，故有学者主 张从2.5mg,d开始，逐渐加至10,20mg,d。 西酞普兰(citalopram，喜普妙，Cipramil)。1997 年Wade等采用双盲对照方法用本品(20或30mg,d)和 氯米帕明治疗475例，两药均优于安慰剂。 非SSRI类药物 文拉法辛(venlafaxne，博乐欣，缓释胶囊名怡诺 思)，Pollacic等经小样本双盲对照研究证明对PD的效果 优于安慰剂。 马氯贝胺(moclobemide)为选择性可逆性MAO,A 抑制剂。但对PD的疗效不佳，不及苯乙肼。 广泛性焦虑症(GAD)的药物治疗进展 如果焦虑的症状较重，心理治疗无效或患者处于较 强的应激之下，使用适当的药物治疗焦虑以改善患者的 生活质量是合理的。临床医生应当在评估时特别注意下 述几点，以便决定选择何种药物及治疗的疗程:(1)焦 虑是短期内出现的，还是慢性持续的,前者应限于短期 用药，后者常需持续治疗。(2)焦虑时以身体症状突出， 还是以精神性焦虑为主，或有体症状与精神焦虑两者并 存,以激动不安突出，还是精神疲乏无力,(3)对焦虑 继发于某些身体疾病的焦虑综合征患者，应查明躯体疾 病的情况，尤应注意肝功能、心血管功能以及有无前列 腺肿大、有无青光眼等。(4)患者的人格特征如何,过 分胆怯、对人缺乏信任、惧怕药物不良反应的人，可能 对治疗不顺从，缺乏自信、情绪不稳、依赖性很强的人， 容易出现用药过度，药物依赖。临床医生还应掌握各种 药物的性能及优缺点。然后才能提供比较合理的治疗。 BZD类药物 20世纪60年代起，BZD类药物取代巴比妥类成了 广泛使用的抗焦虑药。由于其显著的抗焦虑、催眠作用， 起效快、安全性高、不良反应少的特点，长期被视为焦 虑症的首选药。临床经验表明，BZD类药物对伴有身体 症状的焦虑特别有效。1999年对73位药物治疗专家 应用BZD类药物的国际性研究表明，BZD类药物的滥用、 依赖和瘾病的危险低于巴比妥类和其他药品;但也确实 存在依赖和滥用的危险。笔者曾多次见到有些GAD患 者长期服用数种BZD。稍有减量则症状加重，也有的患 者长期轮流使用不同的BZD，由于反复停药，引起焦虑 失眠反跳，症状不断波动，成为慢性患者。BZD类药物 的依赖是"治疗剂量的依赖"，而且有时可引起过度镇静、 精神运动性损害、学习与记忆困难。因此，现在专家们倾 向BZD类药应限于短期使用，停用前须先缓慢减量， 并且由于它们有加重老年人认知障碍和运动失调的可能 性，故用于老年患者时应谨慎。 三环类抗抑郁药 三环类抗抑郁药(TCA)丙米嗪、氯米帕明、多塞平 (doxepin)、阿米替林(amitriptyline)和四环类马普替林 (maprotiline)也有一定的抗焦虑作用。Kahn等(1986) 用丙米嗪和BZD，安慰剂进行对照研究，最初2周以BZD 并不是立即出现，常需要经数周才发生。TCA类药物虽 无依赖性，但副作用较大，患者难以耐受，应先用较小的 剂量，视情况缓慢加量。 SSRI类药物 SSRI类药物由于其高度选择性，副作用少，安全性 高，抗抑郁疗效显著，已成为治疗抑郁症的一线用药。前 已达及有大量双盲对照研究肯定了SSRI类药物治疗惊恐 障碍的价值。近年来也有一些开放性研究证明帕罗西汀、 舍曲林对GAD有肯定疗效。但需要注意突然停服可 引起停药反应，出现头晕，头痛、焦虑、失眠等不适症状。 非SSRI类新药 文拉法辛(博乐欣、怡诺思)是5,HT和NE再摄取 抑制剂(SNAR1)，1999年Roerig回顾了1975年至1999 年治疗GAD的对照研究。认为文拉法辛在短期和长期治 疗中对GAD均有效。建议将文拉法辛作为治疗GAD的 一线药物。现在FDA已经批准GAD为文拉法辛的适 应证。剂量应从小量开始(博乐欣25-50mg/d，怡诺思 37.5mg/d)，视具体情况缓慢增量。曲唑酮(trazodone， 美抒玉)和尼法唑酮(nefszodone)是SRI(5,HT再摄取 抑制剂)和5,HT受体拮抗剂，2研究证实对GAD有效。 2000年戴志萍、张诚、王世刚等报道了曲唑酮治疗GAD 60例的结果，有效率达83.3%，认为曲唑酮是新型高效、 对不适于使用抗抑郁药，又对BZD有依赖者，可选用本 品。 黛力新(Deanxit)为含有两种成分的复方，美利曲 辛为抗抑郁成分，可阻断单胺类神经递质再摄取，氟哌 噻吨为神经阻断成分，阻断突触后D、D和突12触前D， 受体因其含量很小，不起抗精神病作用。黛力新具有抗 轻度抑郁、抗2 焦虑、抗疲乏和镇痛作用。适用于GAD和 "神经衰弱"等。一日2片，早餐后服。但对严重抑郁、惊
