AmJSurgPathol,2010,34:844—51. 去分化脂肪肉瘤的去分化成分有着广泛的组织学变异, 这些变异包括肿瘤的异源性分化.已经证实去分化脂肪肉 瘤有肌纤维母细胞的分化,但是尚未见去分化脂肪肉瘤酷似 炎性肌纤维母细胞肿瘤的报道.作者研究了6例具有炎性 肌纤维母细胞肿瘤特征的去分化脂肪肉瘤的临床病理,免疫 组织化学以及分子学所见.肿瘤多发生于中年或老年男性, 多累及腹股沟/阴囊或腹膜后,且具有侵袭性.光镜下,如果 不考虑背景,这些去分化的成分极像炎性肌纤维母细胞肿 瘤,甚至与炎性肌纤维母细胞肿瘤很难区分.炎性肌纤维母 国外期刊文摘? 细胞肿瘤的3个主要构型(黏液样型,富于细胞型和少细胞 纤维型)都有出现.类似于结节性筋膜炎和纤维瘤病的区域 常见.1例出现了异源性骨分化.4例类似于炎性肌纤维母 细胞肿瘤的区域呈弥漫分布,而其中2例组合非炎性肌纤维 母细胞肿瘤样高级别肉瘤.5例表达SMA或desmin,不表达 ALK.1,5例表达MDM2,通过荧光原位杂交5例显示MDM2 的扩增.每一肿瘤都含有高分化脂肪肉瘤的成分.所有病 人都有肿瘤复发或者转移.最终随访,2例病人死于此病,2 例病人带瘤生存.去分化脂肪肉瘤可以有明确的炎性肌纤 维母细胞肿瘤样特征,该特征进一步扩展了脂肪肉瘤的组织 学谱系.熟知这种特征能避免将去分化脂肪肉瘤误诊为侵 袭性更低的炎性肌纤维母细胞肿瘤.
