在上呼吸道链球菌感染(急性咽峡炎、扁桃体炎)及猩红热时，可并发急性非风湿性心肌炎。其发病机制尚未明了，可能是由链球菌毒素引起。病变是间质性心肌炎。镜下，心肌间质结缔组织内及小血管周围有淋巴细胞、单核细胞浸润，心肌细胞有程度不等的变性、坏死。 三、寄生虫性心肌炎 寄生虫性心肌炎(parasitic myocarditis)常见的有以下两种类型: 1(弓形虫性心肌炎 近来，一些青年人患心肌炎较多，其中最常见的是病毒性心肌炎，其它还有细菌性(白喉等)真菌和原虫等。这些病毒和细菌直接侵犯心肌，以及对心肌内小血管的损伤，免疫机制产生的心肌损伤和毒素对心肌的损害，造成心肌细胞的溶解，间质水肿，单核细胞浸润。如进行病理学切片检查，镜检可见心肌纤维之间与血管四周的结缔组织中可有组织细胞、淋巴、嗜酸或中性粒细胞浸润和间质水肿，心肌纤维可有脂性、颗粒性或玻璃样变性，也可有心肌溶解或坏死。
