手三针和足三针 手三针 足三针 ,张显成, 笔者研究运用手三针、足三针治疗颈肩腰腿和三叉神经痛等疑难痛症近30年～其收效之神速～效果之确切～可谓是:入针即效～拨针即愈。从众多的颈肩腰腿痛、三叉神经痛、顽固性头痛、牙痛等症的治疗观察中～大多能足三针1，2分钟之内收到显效或治愈。笔者应有关单位邀请～曾多次举办全国性和地方性的学习班～听课者达200余人次～遍布20多个省市。但以前所办的学习班～其内容大多是骨质增生症的治疗。在讲解其治疗方法时～只是粗略讲一下手三针、足三针的施针方法～没能从手三针、足三针的作用机理去进行深入细致的探讨。学员听了之后只会知道怎么样去用手三针、足三针治疗颈肩腰腿痛～对其来历和作用机理不太了解。因此笔者感到有必要对手三针、足三针在治疗颈肩腰腿痛、三叉神经痛、顽固性头痛、肋间神经痛等疑难痛症方面的作用机理作进一步的探讨～以求得到同行的指正和惠助～并希望让手三针、足三针的运用得到普及与提高。 一、手三针、足三针的来历 手三针、足三针是笔者足三针近30年的针灸临床工作中～根据其对颈肩腰腿痛等痛症的确切疗效而提出命名的。 二、手三针、足三针各是哪几个穴位 手三针是:后溪、中渚、间谷,笔者发现的一个新穴,。足三针是:太冲、内庭、足临泣。 现将各穴的适应症简述于下。 间谷穴:这是笔者在临床工作中发现的一个新穴～位于手阳明大肠经之三间与合谷两穴连线之中点。它的适应症是～凡三间、合谷两穴的适应症均是其适应症。尤其对颈肩肘臂指部的疼痛麻engineering construction and acceptance specification for 5 GB50275-98 compressor, fan and pump installation engineering construction and acceptance specification for lifting 6 HG20201-2000 construction installation engineering construction standard 7, HG ... (4) to conceal the accident, reported without undue delay or false, to inform the 木～疗效甚为突出。现将《针灸聚英》关于三间、合谷两穴的主治范围摘引如下: 三间:主治喉痹～咽中如梗～下齿龋痛～嗜卧～胸腹满～肠鸣洞泄～寒热症～唇口干～气喘～目眦急痛～吐舌～戾颈～喜惊～多唾～急食不通～伤寒气热～身寒结水等17种病症。 合谷:主治伤寒大渴～脉浮在～发热恶寒～头痛脊强无汗～寒热症～鼻衄不止～热病汗不出～目视不明～生血翳～头痛～下齿龋～耳聋～喉痹～面肿～唇吻不收～喑不能言～口噤不开～偏风～风疹痂疥～偏正头痛～腰脊内引痛～小片单乳蛾等22种病症。 间谷穴之所以能治上述两穴之病症～是因为其穴在两穴连线之中点～其敏感度较两穴高得多。用指甲或其他小棒如火柴杆、圆珠笔头去按压该点～即会感酸胀～用同样的方法去按压合谷其酸胀度就会感到差一些～三间就更差了。敏感度高的穴～得气快～疗效速～似乎无需争议的。 中渚:位于手少阳三焦经的循行线上～在手背第四、五掌指关节间后方凹陷处。其主治～《针灸聚英》谓:“热病汗不出、目眩头痛～耳聋～目生翳膜～久疟～咽肿～肘臂痛～五指不得伸屈”等～《医宗金鉴》谓:“四肢麻木～战振～蜷挛无力～肘臂连肩红肿疼痛～手背痈毒。” 后溪:位于手太阳小肠经的循行线上～第五掌骨小头后方尺侧～手小指外侧本节后陷中。其主治～《针灸聚英》谓:“疟寒热～目赤生翳～鼻衄～耳聋～胸满～头项强不得回顾～癫疾～臂肘拘急～痴疥。”《医宗金鉴》谓:“手足拘挛～手足颤摇不能握～中风卒然昏仆～不能言语～癫痫不省人事～瘈疭抽掣～头痛及暴发火眼～热泪常流～行痹～腿膝腰背历节周身疼痛～项强～伤寒感冒～汗不出～不能解～牙齿腮龈及咽喉肿疼～手足麻痹～破伤受风～寝汗。” engineering construction and acceptance specification for 5 GB50275-98 compressor, fan and pump installation engineering construction and acceptance specification for lifting 6 HG20201-2000 construction installation engineering construction standard 7, HG ... (4) to conceal the accident, reported without undue delay or false, to inform the 以上三个穴位均在手掌背侧部～且系手三阳经的循行线上～分取之可治各自的主症～合而用之更有协同作用。或三穴同用～或两穴同用～或与足三针之一两穴同用～对于颈、肩、臂、肘、手指～及背、脊、腰、胯、腿、足、趾等部位疼痛麻木疗效甚为理想。笔者在治疗上列部位的痛症肿胀～或头痛、牙痛、偏头痛、胁肋痛,肋间神经痛、胆囊或肝部疼痛,,常用手三针、足三针～往往有拔针而愈的效果。 这三个穴位的取穴方法均以握拳少虚为度。 下面谈谈足三针。 足三针:足临泣、内庭、太冲今就三个穴的各自适应症分述如下。 足临泣:位于足少阳胆经的循行线上～在第四、五跖骨结合部前方凹陷处。其主治～《针灸资生经》谓:“主胸中满～缺盆中及腋下马刀疡瘘～善啮颊～天牖中肿～淫泺～,骨行,酸～目眩～枕骨合颅痛～洒淅振寒～心痛～周痹痛无常处～厥逆气喘～不能行～痎疟日发～妇人月事不利～季肋支满～乳痈。” 《医宗金鉴》临泣穴～主治歌:“中风手足举动难～麻痛发热筋拘挛～头风肿痛连腮项～眼赤而痛合头眩～齿痛耳聋咽肿症～游风瘙痒筋牵缠～腿痛胁胀肋肢痛～针入临泣病可痊。” 内庭:位于足阳明胃经的循行线上～第二、第三蹠趾关节前当足次趾外间凹陷中。其主治～《针灸聚英》谓:“四肢厥逆～腹胀满～数欠～恶闻人声～振寒～咽中引痛～口喎～齿龋～疟不思食～脑皮肤痛～鼻衄不止～伤寒手足逆冷～汗不出～赤白痢。”其中并没有关于治疗颈肩腰腿痛等内容。但笔者在临症中单刺内庭治疗大、小腿的疼痛麻痹,特别是前侧,～足面的肿痛、足趾痛麻等症效果特别突出。 太冲:位于足厥阴肝经的循行线上～在第一、第二蹠骨的骨 engineering construction and acceptance specification for 5 GB50275-98 compressor, fan and pump installation engineering construction and acceptance specification for lifting 6 HG20201-2000 construction installation engineering construction standard 7, HG ... (4) to conceal the accident, reported without undue delay or false, to inform the - 3 - 间隙中～当大趾本节后1寸5分凹陷中～以指轻按有动脉应指。其主治～《针灸聚英》谓:“主心痛脉弦～走黄～瘟疫～肩肿吻伤～虚劳浮肿～腰引少腹痛～两丸骞缩～溏泄～遗尿～阴痛～面目苍色～胸胁支满～足寒肝心痛～苍然如死状～终日不得息～大便难～便血～小便淋～小肠疝气痛～溃疝～小便不利～呕血呕逆～发寒～嗌干善渴～肘肿～内踝前痛～淫泺～,月行,酸～腋下马刀～疡漏唇肿～女子漏下不止～小儿卒疝”等多种病症～大致有7种与本文有关。太冲对大小腿内侧的疼痛麻痹、足肿筋挛～腰卒然疼痛等痛症单刺之亦有良好的效果。 纵观以上手三针、足三针的6个穴位～除内庭一穴没有关于颈肩腰腿病的直接记述以外～其他5个穴位均有着较为突出的记述。 据笔者体验～内庭的主治病证～不仅仅是上述那么多～其对大、小腿前侧～足面、足趾、足心、内外踝等部位的疼痛肿胀～拘急麻痹疗效比较突出～比古医籍列述的主治病证效果更为理想。笔者在临床中发现内庭的主治病证～在针灸学教材中未见记述,而《简明中医辞典》,1988年3月～人民卫生版,足阳明胃经条目下有“……颈肿……经脉所过处痛”的记述～虽未能明言病症～但也有所提及～笔者将关于内庭穴的肤浅体会写出来～供同道验证～这对于用手三针、足三针治疗颈肩腰痛等疑难痛症～当有一定的参考价值～ 三、手三针、足三针的适应症 任何一种治疗方法都有其一定适应症范围～手三针、足三针也是这样。 颈肩腰腿痛的发病原因比较复杂～有的也查不出发病原因～甚至用CT、核磁共振等先进的检查手段也查不出病因。找不出原因不等于没有病。笔者认为～只要是人休自身感到疼痛或不适～engineering construction and acceptance specification for 5 GB50275-98 compressor, fan and pump installation engineering construction and acceptance specification for lifting 6 HG20201-2000 construction installation engineering construction standard 7, HG ... (4) to conceal the accident, reported without undue delay or false, to inform the 即是身体发生了疾病。中医是辨证施治～找不出原因不等于不能治。就笔者的体会～手三针、足三针对90,以上的颈肩腰腿痛有突出的疗效～对三叉神经痛、肋间神经痛、头痛、牙痛的疗效尤为突出。不过有一小部分的颈肩腰腿痛用手三针、足三针治疗效果却并不确切～这里所说的疗效确切是指治愈或基本上治愈～效果不确切～不等于没有疗效。比如服止痛药～痛了就吃～药力散去又痛～能说止痛药无效,可以说止痛药有效～只能止止痛～而消除不了病因。手三针、足三针对颈肩腰腿痛止痛效果是理想的～应当说这也是疗效。 四、手三针、足三针治疗颈肩腰痛的作用机理 手三针、足三针的作用是通过经络传感的了解其作用机理首先应了解手足三针的6个穴位所在经脉之循行路线。手、足三针的6个穴～手三针位于手的三条阳经线上～足三针只有太冲位于足厥阴经线上～其他两个穴位均位于足之阳经线上。手三阳经的走向是从手指?手背?前臂?大臂?肩?颈?头面～与足之三阳经相接续。足之三阳从头到足～阳明行于前～少阳行于侧～太阳行于后。足三阴从足到腹～手三阴从胸到手～这样循环无端～息息相通～联络脏腑肢节～沟通上下内外～调节身体各部组织器官的通路～使人体成为一个有机的整体。 关于手三针、足三针～即手三穴、足三穴的经脉循行线路见图。 作者曾作过一个统计:手足三阳经共218个穴～可以用来治疗颈肩腰痛的就有130个,手足三阴经共91个穴～可以用来治疗颈肩腰腿的就有46个:督脉28个～可治以上病症者有20个～任脉没有穴可治上述病症。全身合计大约有196个穴可以治疗颈肩腰腿的疼痛麻痹。若再加上奇穴、天应穴、耳穴等～真是难以计数～这么多的穴位～临床使用起来真是够夏杂的了。 engineering construction and acceptance specification for 5 GB50275-98 compressor, fan and pump installation engineering construction and acceptance specification for lifting 6 HG20201-2000 construction installation engineering construction standard 7, HG ... (4) to conceal the accident, reported without undue delay or false, to inform the - 5 - 笔者在临证时凡见到上述诸症状～不论其发病原因～均先以手三针、足三针进行治疗～一般的不加配穴。当然～就用这6个穴位的适应证～可能不如196个穴位之广泛,但就笔者的经验来讲～上述范围的病症绝大多数可以收到明显的效果。 历来的针灸大家～无不强调针刺手法的重要性。即使辨证准确～选穴得当～而针刺手法不合疾病的法度～也难收到预期的疗效。针刺手法～是一个高难度的技巧问题～不是光凭书本上讲的～或者他人口说的什么手法就能轻易掌握的。一个针灸医生要想掌握住古代针灸大家所陈述的那么多的灵活多变的针刺手法～并达到娴熟自如的境地～必须通过长期的临床实践～细心体会～潜心领悟才行。真是谈何容易:针刺手法可见得是一个十分复杂的技巧问题。据历来的针灸书籍所介绍的手法甚多～一般的临床针灸医生很难掌握住其运用技巧。手三针、足三针虽然也讲究手法～但是却很简单～只要是针灸医生或略知道一点针灸的人都可以很快地学会并能运用于临床。 五、手三针、足三针具有以下特点 1、取穴少。手三针、足三针取穴数最多也只是手足各三针。但是在一般情况下只取1，3个穴即可～手三针足三针同用者极少极少。 2、进针快。进针的速度快如闪电～一般不超过l秒。 3、手法快。针如闪电一样进入穴内～手法要如疾风飞箭一徉。从进针到手法完成约几秒种。 4、得气快。手法快的特殊效应就是得气快～手法到时立即得气～酸麻重胀应针而至。 5、收效快。入针即效～快者几秒钟慢者10余秒钟～就会感到疼痛有所减轻。 6、针刺时间短。针刺时间的长短～是与疗效的有与无、好与engineering construction and acceptance specification for 5 GB50275-98 compressor, fan and pump installation engineering construction and acceptance specification for lifting 6 HG20201-2000 construction installation engineering construction standard 7, HG ... (4) to conceal the accident, reported without undue delay or false, to inform the 差有关。收效显著～即可出针～针感差没效果时间就可稍长一点。笔者在运用手三针、足三针时从入针到出针多在1分钟左右～2，3分钟者较少。 六、手三针、足三针的具体手法 手法问题～包括针前准备、刺入、行针得气、出针等几个具体的程序。 1、针前准备～针手三针时～要向患者讲明～解除其紧张心情～对从未刺灸过的人～更要说明～不痛～几秒钟便可基本解决问题。患者握成虎拳～稍松弛一点,拳头过紧～进针不畅利～不仅会加重疼痛～疗效也不好,太松了针感差～收效相对亦差。待患者握成拳头后～术者用筷子粗细的小棍～从虎口的一侧轻轻穿过尺侧即为合适的握拳。 2、穴位常规消毒后～医生的右拇、食指摄紧针体的锋端～使针尖露1，1.5厘米～先以左手拇指端稍用力向穴位点按即提起～右手之针迅疾刺入～后溪、间谷均垂直刺入～中渚倾斜30度刺入～足三针均倾斜30度刺入。 3、行针得气。针体刺入到一定的深度后～医生右手拇、食、中三指摄住针柄迅疾提针到皮下～又快如飞箭一样完成另两穴的行针路线～针感即至。 4、行针得气后～医生松开持摄针柄的右手指～令患者活动～如颈痛～做摇头、勾头、后仰,肩臂肘痛～做手臂的各种活动～腰痛～做弯腰、侧弯等动作,腿痛做腿的各种活动～等等。一般是行针得气后疼痛即减轻甚或消失。待活动到痛的姿势时～令患者停止活动～并保持痛的姿势～医生把针或提或左右轻旋～问是否减轻或消失～当一个痛的姿势消失～还令其活动寻找疼痛的姿势。从行针得气到患者活动找疼痛的姿势～到收效出针一般是1，2分钟。如一针即生效止痛～一针即可,如一针疗效不显著～可 engineering construction and acceptance specification for 5 GB50275-98 compressor, fan and pump installation engineering construction and acceptance specification for lifting 6 HG20201-2000 construction installation engineering construction standard 7, HG ... (4) to conceal the accident, reported without undue delay or false, to inform the 止痛者可配合同侧翳风穴～多可立即生效。附翳风穴之针法:第一支三又神经痛～直刺得气后提针至皮下再将针尖向上斜刺,第二支直刺:第三支～直刺得气后提针皮下～再偏向下斜刺。 ,十四,顽固性头痛头昏 27、甘某～女～27岁～深圳不锈钢厂职工。自诉:l0年来频频头痛头昏～天气变阴前及下雨天头痛头昏加重～剧烈时心中干呕而不吐～其它均属正常。1993年4月18日21点叩门求治～这次头痛甚剧～一天未止～因是星期日～卧床一天～上午仅喝点稀汤～晚饭没吃。给针右后溪～头痛立止,令其闭是双目～慢慢呼吸～把右手提高过头～又慢慢行轻提慢刺之法～头脑感到清爽而出针。共针5次而彻底痊愈～6个月来下雨刮台风也未再疼痛过。 针后溪治头痛头昏或眩晕疗效甚好～先针一侧～右侧痛针右后溪～左侧痛针左后溪～前额、正中、枕后可针双侧。疗效仍不显著者可先针双后溪再针哑门～怕针哑门有危险者针大椎亦可。如此配合～无不立竿见影矣。 八、关于手三针、足三针疗效机理的探讨 传统的针灸疗法～一般是在辨证的基础上先确定主治穴位～次列配穴～再拟定针刺手法。如治肩周炎～有的针灸医生取肩贞、肩髃、肩井～再配以合谷、外关、手三里等,有的则取肩贞、臂臑～配以合谷、外关、曲池等,入针以后一般都是让患者安静的坐着,笔者称之为静针,～每隔三五分钟行针一次～或再加以温灸～或加以电针器～过20，30分钟起针。而用手三针治疗肩痛～如肩前痛只针间谷～肩后痛只针中渚或后溪～整个肩部均痛者～针间谷、中渚～或配后溪～针后一定要让患者活动～寻找疼痛的角度和姿势～而无需留针～一般不会超过2分钟。 笔者经过数以万次的观察～发现用静计,即让患者坐着不动针刺,治疗颈肩腰腿痛的疗效～远不如动针,针后让患者带针活 engineering construction and acceptance specification for 5 GB50275-98 compressor, fan and pump installation engineering construction and acceptance specification for lifting 6 HG20201-2000 construction installation engineering construction standard 7, HG ...
