辽宁医学院附属第一医院住院总医师(试行) 附件1 辽宁医学院附属第一医院 住院总医师管理制度,试行, 为提高医疗质量、确保医疗安全～通过住院总医师的阶段培养达到提升医师业务技术水平和行政管理能力的目的～结合医院实际情况～特制定辽宁医学院附属第一医院住院总医师制度。 第一条 各临床科室必须设置住院总医师,原则上任期为每人一年,～住院总医师不能代替科室一线值班医师～住院总医师要按要求申报～申报时间为每年6月下旬和12月下旬～科室内设立住院总医师值班室。 第二条 准入制度 1、必须由在临床工作3年以上,博士毕业临床工作2年以上,～且在本院从事临床工作1年以上的注册医师担任。 2、申请人必须医德高尚～无违纪违规行为～无重大医疗差错和医疗纠纷记录。 3、上岗前～要认真填写住院总医师上岗申请～科主任签字后上报医务部审批、备案。医务部组织考核专家小组对住院总医师进行岗前考核～合格者方可上岗工作～考核分医德医技和病历书写两部分考核。 第三条 工作职责 1、在科主任领导下及上级医师指导下～协助科主任做好科inside, and lateral tie circle hooks all the flat to prevent exposure and avoid rust spots in the concrete surface. Concrete blocks: columns, wall structure suitable for color of concrete and plastic clamping 内各项医疗业务和日常的医疗、行政管理工作。 2、住院总医师实施一周五天24小时工作制～周五晚5点到周日晚5点为探家休息时间～此时～各科要由主任指定主治医师以上的人员代班～接替住院总医师工作。住院总医师值班时必须24小时在岗～随时保持通讯畅通～不得离岗脱岗。 3、住院总医师按照相关文件规定负责科内医师的排班,外科住院总医师负责科室各医疗组手术安排～并提交科主任签字。 4、住院总医师负责院内的科间会诊～急会诊10分钟内到位～普通会诊24小时内完成～严禁电话会诊,特殊情况除外,～并做好会诊记录～如遇较为疑难的会诊～或同一病人经两次会诊后仍未明确诊治意见的～应立即如实汇报科主任～由科主任安排副高级职称以上医师处理。 5、住院总医师不具体分管病人～不参加择期手术,20张床位以下科室的住院总医师可以分管床位和参加择期手术～分管床位数应低于科内其他管床医生的平均床位数,～参加手术时～资历相同或以上的人员代为履行职责。 6、协助科主任加强对轮转医师、进修医师和实习医师的日常管理工作,每周一次带领进修、实习医师开展小讲痤。 7、参加和组织疑难危重病人的科内会诊、抢救和治疗工作～带领医师做好早交班、晚查房和巡视工作。 8、按时参加医务部组织的住院总例会～并将会议精神及时传达给科主任及科室医务人员。 inside, and lateral tie circle hooks all the flat to prevent exposure and avoid rust spots in the concrete surface. Concrete blocks: columns, wall structure suitable for color of concrete and plastic clamping 9、每周定期检查科室病历质量～并做好登记和记录,参加医务部组织的病历互查～促进全院病历质量的提高,督促科内各项医疗和技术操作规程的贯彻执行～发现问题责成相关医师整改～相关医师必须服从～各科主任必须支持,掌握病床周转率、床位使用率、平均住院天数、医保等指标完成情况～及时通报科主任。 10、每周一次检查各医疗组合理用药情况～对出现的问题及时科主任,认真核查交班记录～发现问题时～责成相关医师立即纠正～并及时向科主任汇报,组织病房死亡病例讨论、疑难病例讨论、术前讨论～并将结果记录到相应的记录本。 11、督促科室认真、及时地完成每日传染病报卡和院内感染病例等的上报工作～杜绝漏报、延报的发生。 12、协助病案室做好病历的归档工作～督促各医疗组在患者出院后3天内,死亡病例7天内,及时将病历归档。 13、协助科主任及时做好本科室各类投诉、纠纷的接待、处理工作。 14、完成其他各项指令性任务。 第四条 待遇与奖惩 1、住院总医师在科主任不在时履行部分科领导职责,可参加科室中心组会议～可参与科室经济和行政管理, 2、医院设立住院总医师岗位津贴～每人每月200元～就餐补助和通讯费100元～科室在奖金分配上给予奖金系数为1.3。 inside, and lateral tie circle hooks all the flat to prevent exposure and avoid rust spots in the concrete surface. Concrete blocks: columns, wall structure suitable for color of concrete and plastic clamping 3、任期内～医务部进行不定期检查～了解其工作表现并记录在案。任期结束后～个人进行自我鉴定～科主任做出总体评价。 4、医务部根据其平时工作表现、考核情况及科室意见～作出“优秀、合格、不合格”的评定结论～作为本人晋升职称、评优的依据。本科学历的住院医师聘任主治医师或硕士以上学历医师聘任副主任医师时～必须有担任住院总医师半年经历～且考核结果为合格以上～否则不得聘任。 5、医院将根据各住院总医师的工作表现～每年度评选出3—5名优秀住院总医师并给予奖励～在职称聘任时适当加分,对于年度考评不合格者将住院总医师任期延期半年～并再次予以考核。 6、住院总医师在值班期间脱岗或对其工作有不负责行为～查实一次将给予50元罚款～两次将扣除当月总住院医师各项津贴～三次将取消总住院医师资格。 7、在担任住院总医师期间发生医疗纠纷或医疗事故将按医院有关规定处理。 8、在担任住院总医师期间发生医德医风问题将按医院有关规定处理。 第五条 本规定自下发之日起施行～由医务部负责解释。 inside, and lateral tie circle hooks all the flat to prevent exposure and avoid rust spots in the concrete surface.
