大连市初中学生综合素质评价方案

大连市初中学生综合素质评价 (修订稿) 遵照《教育部关于积极推进中小学评价与考试制度改革的通知》精神，依据教育部《关于初中毕业生综合素质评定的指导意见》，结合大连市初中教育实际情况制定本方案。 一、指导思想 1、评价的目标、内容、方法、制度等应适应素质教育的要求，有助于推进基础教育课程改革。 2、坚持科学评价的基本原则，关注学生的全面发展与体现个体差异和特长相结合。 3、积极探索合适的方法和规律，保证评价的公平与有效，不断提高评价工作的专业化水平。 4、不断完善评价的内容、、方法和程序，加强公示、监督、申诉等方面的制度建设，力求各项工作的实效和公正。 5、鼓励各区(市、县)和学校根据自己的实际情况创造性地制定具体的实施方案。 二、实施评价的基本原则 1、多元智能观的评价原则:以多元智能理论指导综合素质评价 ，实行多样化的评价方法，使学生的潜能得以充分表现。 2、评价活动与日常教育活动相结合的原则:评价本身应以帮助学生为主要目的，要尽量消解评价与学习过程的界限。 3、选择性原则:在评价标准的等级设定上，出一定的选择空间，即并不要求学生所有项目全选A，才是A，而是给出一定的余地。 4、实证性原则:利用档案袋把学生平时的表现以及代表作品加以记录并保留下来，使学生综合素质评价尽量有事实依据。 5、量性与质性评价相结合的原则:对学生综合素质评价的年终评价结果作量化(等级)评价，对学生综合素质的过程评价主要采用的是质性评价。 1 三、评价方法体系的确定 具体要运用的评价方法模式如下: (1)观察评价——是在日常教育、教学活动或日常生活等自然条件下，获得对被评价者比较全面、客观的了解。观察评价的内容范围应确定在那些比较明显而易观察的行为表现上。如:按时参加升国旗仪式、主动给老师问好、课堂经常发言等。 (2)档案袋评价—— 每位学生要建立自己的个性发展档案，作为基本评价工具。档案袋中评价卡的内容是统一的，每位学生对档案袋中的评价卡片要及时填写，定时交流，集中保存，作为每年综合素质评价等级确定和查证的参考材料。如:广泛阅读，坚持做读书笔记，这条的评价就应提供一篇读书笔记或读后感;制定合理的学习计划，应提供一份书面材料;能与同学交流，应有与同学交流的评价卡片记录等。 (3)活动展示评价(情景化评价)—— 学校就要定期提供让学生展示自己素质才华的活动和场合，通过有趣的、场景化、主鲜明的活动吸引学生参加，使大家能直接观察到每个学生平日不太被同学了解的一些素质状态，使所有学生都有机会展示自己的能力、兴趣和特长。如 :评价学生的审美表现力就可以让学生运用文学、音乐、美术、戏剧、舞蹈、器乐、体育运动、计算机网页制作、标本采集、科技制作等各种方式表现自己对美的发现、体验和理解。展示结果一目了然。 (4)作品集评价——作品集评价是由学生自己选择一些自己认为最有价值的、有意义的、质量高的代表性作品。作品集中的作品可以含课内的与课外的，但考试卷子不属作品，不在评价之列。 如:完成一项综合实践活动，就要提供一篇研究性学习报告或多某一专题研究的构想等;有克服困难、战胜挫折的经历或体验，就要提供一篇反映自己成长历程的日记或作文等。 (5)检测评价—— 此方法主要用在评价学生的运动与健康状况上，学生的体态状况怎样,身体机能怎样,运动技能怎样,一检测便可得知，它是学校最常用的评价方式。 四、评价内容及要素内涵 根据《教育部关于积极推进中小学评价与考试制度改革的通知》中提出的基础性发展目标，确定五大方面评定内容:公民素质、学习能力、交流与合作、 运动与健康、审美与表现。 2 目 标 要 素 关键行为表现 1. 按时参加升旗仪式～不无故缺席, 2. 向团组织递交过入团申请书, 政治 3. 学校、班级组织的各项爱国主义教育活动均能参加～且有心得体会等相素养 关材料记录, 4. 参加过学校、班级、学科组织的时事演讲或新闻述评等活动～并有发言 记录。 5. 校园内外主动向师长问好、让路～不顶撞老师, 6. 按时值日～不偷懒、不拖沓～主动帮助父母分担家务～并有家长反馈意 见, 7. 不讲粗话、脏话～不骂人、不打架、不侮辱歧视他人, 道德 公8. 帮助过在学习、生活上有困难的同学, 素养 9. 不私自拿取或损坏别人物品～借东西及时归还～损坏他人物品能赔偿, 民 10. 参加过社会公益活动～且有记录, 素11. 遵守公共秩序～讲究社会公德～不乱涂乱刻～不随地吐痰、不随意扔废 养弃物、乘坐公共汽车能主动让座～不大声喧哗。 12. 参加过法制教育、安全教育活动～并有笔记, 13. 不吸烟、不喝酒、不赌博、不沾染毒品～不进网吧等不宜未成年人活动法纪 的场所～不看不传不健康的书刊、音像制品等, 素养 14. 注重仪表～不烫发、不染发、不佩戴首饰～男生不留长发～女生不穿高 跟鞋, 15. 遵守交通法规～不闯红灯～不违章骑车～不斜穿马路。 16. 有个人发展计划～并能坚持实施～有总结, 个性 17. 有克服困难、战胜挫折的经历或体验, 品质 18. 参加过展示自我的活动。 19( 极参与讨论、课堂经常发言, 学习 20( 博的知识面、能主动思考～并与老师和同学共同探讨一些与学科知识兴趣 相关的社会生活问题, 21(有恰当而明晰的自我奋斗目标或榜样～遇到学习上的困难能努力解决, 学习 22(遵守课堂纪律、认真听讲、坚持做笔记,按时、独立、认真地完成作态度 业, 学23(有事主动向老师请假～不迟到、早退、从不旷课。 习24(制定有针对性的合理的学习计划～科学、合理地利用好学习时间～并 能有书面材料, 学习 能识简单的乐谱～学会发声～能够按要求演唱出歌曲所蕴含的情感, 表艺术活58( 能够运用文学、音乐、美术、戏剧、舞蹈、摄影、剪纸、书法、编织现 动与表等艺术形式展示个人的艺术才能～表现生活的美, 现 59( 按时参加学校艺术类课程的学习, 60( 积极参加校内外各级各类艺术展示活动,如演出、比赛、艺术作品展 示等,或参加学校“器乐、舞蹈、合唱三队”。 以上初中学生综合素质评价内容体系的一级指标由五大项构成。二级指标:公民素养有 18项;学习能力有12项;交流与合作有9项;运动与健康有13项;审美与表现有8 项。 4 二级指标总共由60项内容构成。 在运用该指标体系实施评价时，要认真对每个学生个体的综合素质逐条评价。 五、评价标准 对初中学生综合素质评价结果实行等级评价，即根据学生自评、同学评、师评的结果评出A、B、C、D四个等级，而后再根据各自的权重比例，得出五项素质的最终等级结果。其具体评价标准如下。 (一) 公民素养评价标准: 学生的公民素养分为政治素养、道德素养、法纪素养、个性品质四个评定要素。依据国家、省关于中学生综合素质评定工作相关文件精神及《中小学生守则》、《中学生日常行为规范》，结合我市初中教育现状，每个要素设计了关键评定指标(即关键行为表现)，共18项。 评定等级说明 A等:达到关键评定指标中的14条(含14条)以上规定要求，无校纪处分。 B等:达到关键评定指标中的9—13条规定要求，无校纪处分。 C等:达到关键评定指标中的5—8条规定，有校纪处分但已解除。 D等:有违法犯罪行为或受到校纪处分未解除的。 (二) 学习能力评价标准: 学生的学习能力分为学习兴趣、学习态度、学习习惯、学习方法四个评定要素。依据国家、省关于中学生综合素质评定工作相关文件精神，结合我市初中教育现状，每个要素设计了关键评定指标(即关键行为表现)，共12项: 评定等级说明。 A等:至少达到关键评定指标中的9条规定要求。 B等:达到关键评定指标中的6—8条规定要求。 C等:达到关键评定指标中的3—7条规定。 D等:达到关键评定指标中的3条以下规定 (三) 交流与合作评价标准: 学生的交流与合作能力分为团队精神、沟通与分享两个要素。依据国家、省关于中学生综合素质评价工作相关文件精神，结合我市初中教育现状，每个要素设计了关键行为指标(即关键表现)，共9项。 5 评价等级说明 A等:具备关键评价指标中的任意7——9条规定。 B等:具备关键评价指标中的任意5——6条规定。 C等:具备关键评价指标中的任意 3——4条规定。 D等:具备关键评价指标中的任意 1——2条规定。 (四) 运动与健康 运动与健康能力评定由健康心理、健康体魄、健康生活方式、运动素养四个要素组成，结合我市初中教育现状，每个要素设计了关键评定指标(即关键行为表现)，共13项: 评定等级说明 A等:达到关键评定指标中的10条(含10条)以上规定要求，无心理障碍，体育课、眼操、间操、大课间活动出勤率不低于95%。 B等:达到关键评定指标中的8—11条规定要求，无心理障碍，体育课、眼操、间操、大课间活动出勤率不低于85%。 C等:达到关键评定指标中的4—7条规定，体育课、眼操、间操、大课间活动出勤率不低于75%。 D等:有心理障碍或不健康行为表现，体育课、眼操、间操、大课间活动出勤率低于75%。有以上之一者则为D。 (五) 审美与表现评价标准: 学生的审美与表现分为审美情趣、艺术活动与表现两大要素，每个要素设计了关键评定指标 。 评定等级说明 A等:达到关键评定指标中的 6条，(含 6条)以上规定要求， B等:达到关键评定指标中的 3—5条规定要求， C 等:达到关键评定指标中的1—2 条规定要求， D等:没有达到关键评定指标中的任何一条规定要求。 六、评价建议及注意事项 1、在评价过程中，要做到公平公正，学校应做到对每个评定结果都能做出解释，要有说服力，评价结果通知本人。对“D”等级的评定应非常慎重，要有明确解释和详实的书面 6 材料。 2、评价要点 (1)评价次数:原则上每学期末评定一次，毕业前进行合成。 (2)评定人员:班主任，任课教师、学生。 (3)评定方式:教师评定、学生互评、学生自评。 (4)评定结果:以A、B、C、D四个等级呈现。 (5)学生公民素养毕业评定合成办法: 为了便于以下计算，赋予各等级的分值为: A=4，B=3，C=2，D=1。 3、计分公式: 学期末等级分值=(教师评价等级平均值×40%)+(小组评价等级平均值×40%)+(学生自评等级值×20%) 各等级分值阈限为: 3.5 ?A等?4.0 2.5? B等,3.5 1.5? C等,2.5 D等,1.5 学生公民素养毕业评定合成办法:根据学生初中阶段六个学期的公民素养评定等级分值的平均值,对照等级分值阈限,合成学生公民素养评定等级。 (6)有关说明事项: ?教师评价等级平均值=(4×A的累计人数，3×B的累计人数，2×C的累计人数，1×D的累计人数)?教师人数。 ?小组评价的等级转换成数值直接纳入学期评定之中。 ?学生自评等级转换成数值直接纳入学期评定之中。 4、范例: 初三某班张某，在初中六个学期末的公民素养评定等级分值分别为: 学期 初一上 初一下 初二上 初二下 初三上 初三下 等级 C B B A A A 分值 1.9 2.6 3.1 3.6 3.6 3.8 7 该生初中阶段六个学期的公民素养评定等级分值的平均值为3.1,对照等级分值阈限 3.5 ?A等?4.0 2.5? C等,2.5 D等,1.5 该生的公民素养毕业评定等级为B等。 七、评价的组织机构: 1、为了搞好初中毕业生综合素质评价，各区(市、县)要成立评定委员会。由教育行政部门领导、校长、教师和其他社会人士组成。其职责包括:制定或审核评定的规章制度;监督评定程序;接受和处理各界人士的质询、申诉，纠正评定中的错误、查处违规行为。 2、各学校要成立评价委员会。由校长、教师、学生和其他社会人士组成。其主要职责包括:落实各区评定方案，制定具体的评定程序，认定评定小组成员，组织并监督评定工作，对评定结果进行认定，对评定中出现的分歧予以澄清和仲裁。 3、班级成立评价小组。由班主任、科任教师和学生组成，小组成员不少于9人(必须有艺、体教师参加)。负责组织班级学生日常评价工作，负责学期数据的录入。 八、评价的实施程序 1、评价的准备 评价的准备指的是设计方案、制定规划、编制计划和开展宣传等系列工作。 程序1 设计评价方案 教育评价是按照严格的科学程序进行的一种活动，必须事先设计出一套评价方案，用以规范评价者和评价对象的思想行动，指导和调控评价实施活动的全过程。 程序2 制定评价细则 为使评价工作在较大的范围内展开，并能按照统一的部署进行，应制定具体的评价工作程序，使评价实施活动做到制度化和经常化。每学期末进行一次综合评定(包括自评、小组评、师评)。在制定评价细则中，要规定出评价人员的工作规范、职权范围等。 程序3 开展评价宣传培训 评价宣传工作内容: 8 (1)召开评价动员会。请有关领导做专题报告，进行评价工作的思想动员;组织有关人员学习、讨论、研究各级教育行政部门颁发的有关综合素质评价的各种文件，通过动员，明确评价目的和意义，提高他们对学生综合素质评价的必要性和重要性的认识。 (2)举办综合素质评价专题讲座。请评价专家讲学，普及综合素质评价的理论与技术，掌握实施评价活动的方法和程序。 (3)利用各种可以利用的宣传媒体，介绍评价的实施方案，让学生、家长、教师等明确其意义、内容及具体实施要求。 2、评价的施行 程序4 收集、准备评价材料 每位学生要根据具体评价内容和指标的要求，建立自己的成长档案袋与作品集，按照学校老师的统一要求，有目的、有计划地定期填写评价卡片，把每张评价卡的实证材料写实做实，把评价所需材料准备好。(收集评价档案袋的具体内容附后) 程序5 平时考核与定期反馈评价信息 班级评价小组注重平时对学生的考核，并定期(如1周)向学生反馈评价信息。 程序6 总结立案建档 评价档案资料收集齐全后，要及时进行总结性整理，进行科学分类，统一编号，固定位置，使得档案整体化、系统化、规范化，以便迅速查找所需的材料。 档案案卷目录表 顺序号 案卷标题 填写日期 文件张数 是否交流 备注 1 2 3 4 ? 作品集目录表 顺序号 作品内容 收集日期 作品页数 是否交流 备注 1 2 3 4 ? 程序7 网络操作 9 学期末，由学生本人、班级评价小组、任课教师将过程评价的数据录入电脑程序，上传至网络中心，生成评价结果。 3、处理评价结果 程序8 审查评价质量 学校评价领导小组要对每个班级的评价过程、评价程序和评价结果复核认可，对学生、家长和社会上对评价反映意见较大的问题和缺点，要给以合理地解决和修正。 4、各项综合素质评价指标与评价方法的对照参考 评价指标 主要评价方法和形式(工具) 公民素养 观察评价、档案袋评价、活动展示评价 学习能力 观察评价、档案袋评价、活动展示评价、作品集评价 交流与合作 观察评价、档案袋评价、活动展示评价 运动与健康 检测评价、观察评价 审美与表现 活动展示评价、作品集评价 九、对评价中可能出现的问题及其对策 1、正确引导学生自评是班级管理中用好档案袋评价的关键 学生的自我评价和反思，是档案袋评价的一个重要特点与要求，因此，教师要鼓励和引导学生，依据评价卡的标准和要求评价自己的某方面，反思自己的学习过程，发现自己的优势和不足，形成对自己负责任的态度。为此教师不能只是简单提出要求，还要提供必要指导。 2、慎重评价是年终量化评价的重要环节 在他评中，无论是班主任评、科任教师评、还是小组评，都要遵循慎重评价的原则，要充分考虑到评价所产生的各种后果，要使评价起到最大的激励作用。在评价中要做到以下几点:一是要在评价的前后做过细的思想工作。二是善于在评价中发掘学生的闪光点。三是评价要掌握统一的标准，标准统一，评价才公平、公正，对学生才有说服力。让每个学生都能体会到只要自己在某个方面付出了努力，就能获得公正的、客观的评价。 十、保障措施 1、建立层层指导监控机制。市领导小组负责指导和检查区评定委员会工作，区评定委员会负责指导和检查学校工作，学校评定委员会指导监控评定小组工作。学校评定机构要报 10 上级部门备案。评定小组成员由学校评定委员会在开展评定工作一个月前提名成立，并向班级学生公示，如有1/3以上学生不同意其作为评定小组成员的，则需换用其他合适人选。 2、建立公示制度。初中毕业生综合素质评价的内容、方法、程序及结果的使用等，应用公开的方式向学生、家长、教师作出明确的说明和解释。 学生综合素质评定结果应在所在班级公布，接受学生的质询。应充分考虑并消除公布评定结果对部分学生的所造成的消极影响，可只公布在各项评定内容上等级为A等的学生名单，其余的通过其它方式告知学生本人和家长。 3、建立举报和申诉制度。学生、家长、教师和其他社会人士对于评定过程中可能危害评定结果公平、公正的现象和行为，或者对评定结果存有异议，可首先向学校评定委员会提出举报或申诉，如果对校评定委员会的答复或处理不满意，可向区市县评定委员会提出举报或申诉。上级委员会应详细记录各项举报、申诉及查处过程和结果。 4、建立信用记录。评定委员会应为学生综合素质评定者(包括教师、学生)、学校、评定委员会建立信用记录。如果在评定过程中出现弄虚作假的行为，则应在相应当事人(单位)的信用记录中予以记载。信用记录将作为高中招生的重要参考，对于有不良信用记录的当事人(单位)，高中有权加大审查力度、减少招生名额甚至拒绝录取。信用记录不佳的教师不能作为评定小组的成员，并且在教师考核中不能评优评先。 5、建立评定质量监控与完善机制。在评定工作进行中以及结束后，学校评定委员会应组织人员对评定质量进行监控和评估，充分了解评定工作的有关信息，及时纠正评定工作中存在的问题并提出改进建议。评定工作结束后评定委员会应进行质量分析并撰写工作报告报主管部门备案。区评定委员会应对各初中评定工作按一定比例抽查，对评定结果的真实性和有效性进行核查，尤其对获得A等级比例过高的学校和班级进行复核，如果其中确实存在弄虚作假的情况则应在信用记录中予以记载，如有重大问题应组织重新评定。高中学校有责任对升入高中学生的综合素质进行跟踪评估，将评估结果与初中的评定结果相对照，并报区级评委会，如果两者出现较为严重的差异，报区评定委员会审核，一经查实评定明显失实的，在相应当事人(单位)的信用记录中予以记载。 二〇〇七年五月十五日 11 (英文版 ) Two regulations promulgated for implementation is in the party in power for a long time and the rule of law conditions, the implementation of comprehensive strictly strategic plan, implementation in accordance with the rules and discipline to manage the party, strengthen inner-party supervision of major initiatives.
