头孢替唑钠不良反应 头孢替唑钠与葡萄糖酸钙存在配伍禁忌 【临床药学讨论版】 患者女,七个月.因发热五天伴抽搐四次来诊.诊断:1上呼吸道感染;2低钙性抽搐.给予10%葡萄糖溶液100ml加葡萄糖酸钙10ml静滴,头孢替唑钠0.4g加管,5min后输液管内出现白色沉淀物,考虑为头孢替唑钠与葡萄糖酸钙混合后出现的沉淀.重新更换输液器继续输葡萄糖酸钙,无不良反应出现,经查<<256种注射配伍变化检索表>>中无记载,望诸位爱友多 多注意. 患者 【通用名称】注射用头孢替唑钠 【英文名称】 【成份】头孢替唑钠。化学名称:(6R，7R)-3-[(1，3，4-噻二唑-2-基)硫代甲基]-8-氧代-7-[2-(1H-四唑-1-基)乙酰氨基]-5-硫杂-1-氮杂双环[4.2.0]辛-2-烯-2甲酸钠。 【性状】本品为白色至淡黄色结晶性粉末，无臭，有引湿性。 【作用类别】 【药理毒理】头孢替唑钠为半合成的头孢菌素衍生物，其作用机理是通过抑制细菌细胞壁的合成而发挥抗菌活性。头孢替唑钠对下列细菌有抗菌活性:1.需氧革兰氏阳性菌:金黄色葡萄球菌、肺炎链球菌、化脓性链球菌。2.需氧革兰氏阴性菌:大肠杆菌、肺炎克雷伯杆菌、变形杆菌。 【药代动力学】本品在体内分布广泛，其中以肾脏最高，依次为血清、肝脏、肺、心、脾。脑脊液中也有少量分布，但脑膜炎症时分布明显增加。在炎性渗出液中的分布，与头孢唑林、头孢噻啶和头孢噻吩相比，头孢替唑为最高，用药后5小时，在渗出液中仍还有相当量的头孢替唑，明显比血中浓度维持时间长。本品健康成年人肌注0.5g，15-30分钟时血清浓度达峰值，血药峰浓度为24.9μg/ml，维持至4小时约2.6μg/ml，半衰期为56分钟，24小时尿中回收率为86.6%。健康成人一次静滴1g(加 500ml生理盐水)，1小时滴完。在开始静滴后15分钟血药浓度达峰值(38.75μg/ml)，点滴过程中维持此浓度，点滴结束后15分钟血药浓度降至25μg/ml，30分钟为6.25μg/ml，半衰期为0.33小时。 【适应症】呼吸系统感染、泌尿系统感染、败血症、腹膜炎。 【用法和用量】成人一次0.5g,2g ( 2,8支 )，一日2次静脉给药或肌肉注射。重症感染，剂量可按医嘱适当增加。注射液的配制: 1.静脉注射:溶于注射用水、生理盐水或5%葡萄糖注射液，缓慢注射。 2.静脉点滴:溶于生理盐水或5%葡萄糖注射液。 3.肌内注射:溶于0.5%盐酸利多卡因注射液。注射液溶解时如因温度原因出现混浊，可加the needs of future rehabilitation centre. facilitates future expansion of the hospital-wide voice communications, reducing costs due to expansion. Medical network to four-room for 2 large Gigabit switches at the core, double-machine 温使其澄清后使用。溶解后最好立即使用，如需保存，为防止发生沉淀，应在阴凉处(15?以下)保存，但必须在72小时内使用。 【不良反应】1.休克:极少有休克发生，需进行严密观察。当有任何与本品使用有关的症状，如:不适感、口内异常感、哮喘、眩晕、突然排便异常、耳鸣、出汗等症状，应立即停止用药，并予必要处理。 2.少数有过敏反应，如皮疹、荨麻疹、皮肤发红、瘙痒、发热等，出现时停药即消失。 3.罕见严重肾功能异常，偶见血肌酐升高，如发现异常应立即停药，进行必要处理。 4.罕见粒细胞减少、白细胞减少、嗜酸粒细胞增多、血小板减少等，当发现有上述异常时，应停止用药。 5.罕见GOT、GPT、碱性磷酸酶增加，有上述异常时，应立即停止用药。J应名D 6.偶见胃肠道反应，如恶心、呕吐、腹泻等，罕见伪膜性肠炎，当有腹痛、腹泻出现时，应立即停药。 7.菌群失调:罕见发生念珠菌症。 8.维生素缺乏症:罕见发生维生素K缺乏症和B组维生素缺乏症。 9.罕见间质性肺炎、毒理性发热、咳嗽、呼吸困难、胸部X线异常、嗜酸粒细胞增多，当出现上述症状时，应停止用药，并给予相应的治疗。 10.其它:罕见头痛、全身不适感、发热、浅表性舌炎。 【禁忌】1.对本品或其它头孢菌素类抗生素过敏者禁用。 2.对利多卡因或酰基苯胺类局部麻醉剂有过敏史者禁用本品肌注。 【注意事项】1.为预防过敏反应的发生，用药前应详细询问病人过敏史，并进行皮肤过敏试验。 2.肾功能不全者慎用。 3.青霉素过敏者慎用。 4.静脉内大量注射，偶尔可引起血管痛、血栓性静脉炎，故要注意调整注射部位和注射方法。注射速度要尽量缓慢。 5.本人或直系亲属中有易发生支气管哮喘、皮疹、荨麻疹等体质者慎用。 6.对不能很好进食或需接受静脉营养的患者，老年患者，体弱者需严密监测，警惕出现维生素K缺乏症的可能。 7.肌内注射时可发生注射部位疼痛、硬结，故不可在同一部位反复注射。 8.肌内注射时使用的溶剂不能用于静脉注射和静脉点滴。 【孕妇及哺乳期妇女用药】关于妊娠中用药的安全性尚未确定。对于孕妇或有妊娠可能性的妇女，要权衡利弊，只有在利大于弊的情况下才能用药。因此孕妇及哺乳期妇女慎用。 【儿童用药】小儿按每公斤体重一次10,40mg，一日2次静脉给药或肌注(注射液的调配见[用法用量])。 【老年患者用药】 the needs of future rehabilitation centre. facilitates future expansion of the hospital-wide voice communications, reducing costs due to expansion.
